Smaller breast implants definitely have lower complication profile such as bottoming out, capsular contracture and implant displacements; however, I have not seen any association between BII and implant size.
Regarding your second question, it is definitely safe to perform breast augmentation together with tummy tuck. Mommy make over which is a combination of breast procedure with tummy tuck is very frequently performed and it is very safe in slim people like you. I would recommend to consider the surgery with a board certified plastic surgeon.

A breast augmentation and lift would definitely improve the symmetry between the breasts and increase the volume of the breasts at the same time. Due to complexity of the surgery, some may call it reconstructive.
I do not recommend breast augmentation alone as we have to address the asymmetry between the shape, size and location of the nipples. Also there is significant different in the distance between the nipple and lower fold of the breast, which needs to be addressed during the surgery.

I do not think upper body lift would resolve your problem. I am just curious if you have lost some more weight after your tummy tuck surgery, which has lead to some extra skin.
Part of the bulge in upper abdomen is due to the fat around the intestines, which can not be removed by tummy tuck or liposuction. To get rid of that, I suggest to try to lose some more weight.

It seems that you have a breast lift together with augmentation and there has been some healing issues around the areola. One reason might have been due to large implants. Depending on the timing since your surgery, you may regain part of the pigments back, however, it will not be 100%.
I recommend to consider tattooing the areola.
If you decide to change your implants with smaller implants, then you can have a revision of the breast lift and correct part of the scars, however, it will not bring the pigment back to the areola.

I am sorry for your experience with breast augmentation surgery. Based on the provided pictures, it seems that the implant has settled down and the upper pole of the implant is more visible. It is hard to see if the breast tissue has become slightly sagging or not. Specially after pregnancy, it is common for breast tissue settles down even more.
If you had an high profile or ultra high profile breast implant, the upper pole of the implant can become very prominent in slim people which may look like it is pushing the chest wall out.
If it is the case, you can switch the implant to slightly smaller one with less projection and perform a breast lift, which will correct this deformity.
