Dr Ptak is an amazing plastic surgeon who is not only progressive but is also an artist. Over the last twenty years I have always been beyond happy and feel so lucky to have him as my surgeon. As I am aging, I like the options of nonsurgical procedures that help me build my own collagen like Morpheus8, along with FaceTite and fat grafting. These procedures are a game changer for the aging face looking for help with contour and laxity in the lower face. Fat grafting is an added plus to help with achieving natural and subtle fullness. I met with Dr. Ptak twice before finally pulling the trigger on my surgery for Gynecomastia. I could tell right away he was a very educated and intelligent doctor and his accolades sort of speak for themselves. I was very nervous the day of surgery, I elected to do a local anesthesia / painkiller combo route for the surgery (instead of full knock-out anesthesia) and I was wondering about how much pain there would be during the procedure even though I was assured practically none. They were right. It turned out to be not that bad whatsoever. I basically didn't feel any pain except mild (maybe a 3 on scale from 1 to 10) towards the end of the surgery, which Dr. Ptak was always aware of my comfort level and increased local anesthesia when needed. Recovery after the surgery was pretty easy also and after only a month, I'm already happy with the results and I know they will continue to get better through proper post-op care. If you're thinking about getting Gynecomastia Surgery, I would highly recommend Dr. Ptak and also highly recommend that you should decide to do it if it's been weighing on you for years like it did with me. My only regret was that I didn't do this sooner.
Had the surgery almost a year ago and I am sooooo happy with the results. I am a 69-year-old woman very experienced with cosmetic procedures. I feel that I could be considered an expert patient if there were such a thing. After extensive research, I chose Dr. Steve Ptak for my complicated requests. Of course, the fact that he has been on the best doctors list for many years did help with my decision. I found Dr. Ptak to be the kindest and most empathetic doctor I have ever been to. No doctor ever took so much time with me thoroughly explaining my procedure options, potential downsides, and what results I could reasonably expect. I was requesting a lot while stressing my need to stay within my budget. I wanted both arms as thin as possible, reduced back and armpit fat, and lastly a reduction of a thick ugly scar that curved perfectly under my breasts. (This is how they did heart surgery 50 years ago). After surgery, I was so overwhelmed when I first took my bandages off that I actually cried with joy and appreciation. The existing scar was halved and less visible, looking more like a tight bra line. It was my beautiful arms and loss of back fat that thrilled me so. At the six weeks post op exam, I barely mentioned that one arm looked a little smaller and smoother than the other. I could have lived with it because to me it would have been unreasonable to expect absolute perfection considering all that he did that was perfect. This wonderful man immediately scheduled me for the small repairs and didn't charge me a dime! I now knew that this amazingly skillful artist was a perfectionist and didn't want me to leave until I was totally thrilled with my results. I was told to undergo this procedure 3 years ago by my dermatologist. I wasn't ready at the time. This time around my Plastic Surgeon recommended it, stating that I would achieve the results I was looking for without undergoing major surgial procedures. I went into the drs office the late afternoon before treatment. Levulan was applied to my face with very little discomfort. Went home and slept with the product on my face. Went back to the Drs office the next morning to have the blu light therapy. This was the most PAINFUL cosmetic experiecne I have ever had. I was in instant pain. We had to do the treatment only a few minutes at a time, whiched eventually totaled 12 minutes. The pain I experienced was worse than natural childbirth, which I have done. It felt like my face was being torched. Below are before pictures, day of pictures, and day after pictures. I will continue to follow up giving my final review in a few weeks.
I researched several doctors before choosing 5 to interview for my procedure. Yes, doing your home work is crucial. I chose Dr. Ptak for a plethora of reasons. First, he is very thorough in his explanations. Second, his note worthiness in many publications and references. Third, his amazing techniques and staff. This review would be complete if I didn't say how wonderful and caring Marilyn is. I had something tragic happen to me when I was in my teens that caused me to want breast augmentation later in life. From your photos it is really impossible to realistically answer your questions without an in person exam and consultation. It appears you have an over-projecting tip and possibly low dorsum (backbone) which makes the tip look larger. Most complex tip operations can still be done under a mild sedation in an in-office surgical suite, with or without some changes to the dorsum, which could be augmented with fillers. Any reputable plastic surgeon will tell you even the most experienced plastic surgeons that have performed hundreds of rhinoplasties feel every rhinoplasty is complex and requires the highest level of expertise.
Acne scars are not all the same and the scarring can extend all the way into the subnormal fat layer depending on the intensity of the infection. Superficial treatments like fractional lasers and micro needling cannot rebuild tissue deeper than 1-2 mm. There are only one or two treatments in my experience of 25 years that can truly regenerate dermal collagen and subdermal volume in one treatment and that is fat grafting under the area of the scars. Intradermal injection of "nanofat" which is the centrifuged and filtered concentrated portion of the fat graft has been shown to be very effective in the actual regeneration of dermal collagen which is far superior to any filler and is an important adjunctive treatment to lasers and micro needling.
Breasts are rarely perfectly matched. Like snowflakes, no two are alike - sisters, not twins. But when there is very significant asymmetry as in your case you need to explore all options with your experienced and board-certified surgeon. Your photos are not adequate to give you ideal advice - you need to seek multiple consultations and opinions, as there are multiple solutions that are possible and probably necessary to achieve a close match. These would include various implant shapes and sizes, release of any constrictions, matching of the areola and skin envelope, and fat grafting. J Ptak MD Scottsdale, AZ
Yes, fat grafting can often make a substantial volume increase in breast size but more importantly be very helpful in improving the overall shape and projection in a small breast. I have been encouraging patients to consider a combination small implant and fat grafting to achieve both the desired volume and a much better shape that one or the other alone can't achieve. Since there is a fairly large variation in the success of fat grafting and the maximum amount of fat that should be grafted at one procedure, I agree that you should only expect a one cup increase from each grafting procedure. From your photos it appears that your implants are too wide at the base for your chest dimensions and your nipple diameter should have been addressed at the initial breast procedure as it is clear from your before photos that they were already somewhat large and then only enlarge further with such large implants. I am personally not a great fan of the circumareolar reduction as it has a high rate of re-stretching to the original size over time and would not be appropriate if you are considering downsizing the implants that will leave you with significant excess skin requiring at least a "lollipop" lift.
