Facials with extractions are fine to do while on Accutane as long as the esthetician helping you is GENTLE. You do not want to be too aggressive, as over zealous extractions could lead to scarring.
Getting a facial can be enjoyable and help you look your best. However, be sure that your esthetician is using high quality products and is gentle with your skin. Too aggressive extractions, etc can cause persistent dark spots on patients with darker toned skin.

Be very cautious about doing anything to traumatize or irritate a keloid scar. It may grow even larger. It is best to find a surgeon with expertise in management of keloids, as in the right hands you could do very well.
A combination of excision (preferably with radio-frequency surgery), and steroid injection is a good start. Postoperative and routine steroid injections can be beneficial as well. You may want to discuss with your surgeon the risks and benefits of healing by second intent (ie: without stitches)
