Getting my surgery done by Mr Rao was one of the best decisions I made. He made me feel completely at ease and informed thought the entire process. I felt he was very attentive and took time for all my questions and queries pre and post op. I couldn't be happier with my results and the use of simulation pictures were really great to see what my results could look like. Having come in with a severely deviated broken nose and I mean severe , Mr Roa truly went above and beyond and has done what people around me are describing as a miracle; a truly exceptional job. I would highly recommend Mr Rao to anybody who is considering facial surgery. In October 2022 I had a tip rhinoplasty, genioplasty, neck lift and buccal fat removal. Mr Rao is professional, knowledgeable and caring. He is approachable and easy to talk to. I was very sensitive about my facial features but I was able to talk to Mr Rao without feeling embarrassed or judged. I have recently had Botox and Filler treatment by Mr Rao. Prominent ears are commonly caused by a lack of the anti helical fold of the ear where it is flattened instead. Enlargement of the conchal/bowl of the ear is the other main cause. You do have a Darwin tubercle, but it is likely better to correct the prominence by folding the ear rather than trying to remove the tubercle, but it can be done. I suggest you see a facial plastic surgeon like myself who can assess your ears and advise you accordingly.
Your setback otoplasty should have been planned tailored to your situation. The use of dissolving stitches has the reduced risk of stitch exposure and feeling it under the skin in the future. Also the reduced risk of infection. My personal opinion is that these are a very low risk in general. My preference is to use permanent stitches and sometimes heavier thickness ones. Using a small fascial flap to cover the stitches provides thicker tissue over them to reduce the risk of exposure and feeling them. If the procedure is done carefully, kept clean and use of antibiotics, the risk of infection is very low. The choice of creating either, anti-helical folds where lost, set back of the concha of the ear, or both depends on the problem with your ear. Use of mustarde stitches for the anti-helical folds or furnass stitches for the concha is my preference. Using both is not always a necessity as it can 'over correct' the ear and cause 'telephone deformities'. Relapse has a higher risk in thick cartilages or if healing is compromised during recovery. The need for a revision due to relapse is not uncommon. Discuss the choices above with your surgeon.
It is hard to tell the if there has been any change or not as the lighting is different between the 2 photos and alignment. Both these can make the ear look more prominent. A before any surgery photo would have been helpful. It can be seen that the before photo shows no shadow in the scapha of the ear, but in the after there is a shadow suggesting that an anti-helical fold has been introduced. The lack of the anti-helical fold is often the issue with prominent ears. Thick cartilage can be weakened to reduce the risk of relapse. Also the use of a different stitch material and ensuring the recovery instructions are undertaken correctly.
You have a reasonably normal chin projection however the photos may not truly reflect the concern due to the angulation. Depending what you are looking for you could go with a temporary reversible solution with fillers to augment the chin. If you were then happy with this you could move to an advancement genioplasty or chin implant depending on your choices and the amount of advancement you need. The jaw advancement surgery will certainly have improved the chin projection and it would need a decision on what more you want from your chin projection.
You are best to see a Maxillofacial surgeon with an interest in cosmetic facial plastic surgery also. It is hard to be sure as the photos provided do not show the full facial profile including the nose. However is does seem you have a retruded chin and its relationship to the lips and nose. The issue could be a retruded jaw, then jaw advancement surgery combined with orthodontics would address the retruded chin problem. If the jaw position is fine then a surgical advancement genioplasty would improve this with or without a rhinoplasty. Alternatively is a chin implant. A full facial analysis is required.
