Prone to blood clots, is getting a tummy tuck safe?

Thank you for the question. Any surgical procedure has the risk of blood clots. Tummy tuck has a higher rate compared with many other plastic surgery procedures, although this rate is still very small. However, pulmonary embolism may be fatal, and this complication should not be taken lightly. 

History of pulmonary embolism is not an absolute contraindication to surgery (in other words, not a hard stop). Assuming you have a low surgical risk otherwise (no nicotine or other drug use, reasonable BMI and goals, etc.), this risk can usually be managed. However, this may require additional blood tests, consultation with a hematologist, etc. depending on the surgeon you visit. In my practice you would, at minimum, require an injectable blood thinner once a day for at least 1 week after surgery.

I strongly recommend consultation with a plastic surgeon board certified by the American Board of Plastic Surgery to discuss your specific case.

Is it better to inject fat deep into the muscle and/or other layers?

Thank you for the question. We do not naturally have fat within muscle itself, and injection into muscle is avoided. Fat may be injected into a deep area, but injection into muscle itself is nonanatomic and not beneficial.

Can something be done about my nose becoming more symmetrical and small without rhinoplasty? (Photo)

Hi, thanks for the question. The short answer is no, your nose cannot be shaped and resized in this area without rhinoplasty. Your nasal tip is made up of a complex 3D framework of cartilage and specialized soft tissue that must be surgically altered to become smaller and more symmetric. After these cartilages have completed growing, there is no taping or external adjustments that can change the form of your nose.

2nd revision wanting to change subglandular to submuscular? (Photo)

How can I treat the wrinkles in my labia majora?

Often wrinkles of the labia majora are caused by atrophy (thinning) of the tissue under the skin. Injection of a small amount of your own fat into the area is my usual recommended treatment for this concern. I highly recommend consultation with a board certified plastic surgeon to discuss treatment.

Can animation deformity after a double mastectomy and reconstruction with LD flap and implants be corrected after 5 years?

Thank you for the question. Animation deformity is related to movement of implants with contracture of the pectoralis major muscle (usually), and sometimes due to continued latissimus muscle contracture (uncommonly). This may be addressed with either attenuation of the pectoralis muscle or denervation of the latissimus muscle, depending on which is causing the issue. However, the implants "looking flat" may be due to other issues besides animation deformity. 

Pain may or may not be related to the muscle activity, and can have many causes. In other words, correcting the appearance concern may not cause resolution of your discomfort. I would strongly suggest discussing this with your original surgeon or seeking an opinion from a board-certified plastic surgeon.

Chin implant stability with applied pressure or blunt force due to martial arts (bjj and boxing)?

Any form of implant, or even a sliding osseous genioplasty with titanium plate/screws may become displaced if enough force is applied to the area. In addition, disruption of the surrounding scar or overlying soft tissue damage could cause infection or exposure of the implant. These complications may occur even in patients who do not do any sort of contact sports.

Anti-Pigmentation Cream for burned Skin? Any advice?

Thanks for the question. Chemical peeling agents, like glycolic acid, produce a controlled damage to skin, much like a partial-thickness burn. This should not cause lasting hyperpigmentation. Rather than applying an additional chemical agent, such as a hydroquinone-based lightening cream, it is best to allow your skin to heal and use gentle acne treatments. Lack of blistering is good, and indicated a very superficial skin injury from the glycolic acid treatment. 

Be careful to use sunscreen continuously to provide another barrier to hyperpigmentation.

Is it possible getting empty nose syndrome symptoms just by getting nasal spreaders grafts?

Thanks for the question. Since spreader grafts by definition decrease the size of the airway (in addition to preventing loss of midvault support), they cannot by themselves cause "empty nose syndrome." This syndrome is typically the result of overreducing the turbinates. A similar sensation could be achieved by a very large septal perforation.

Will breast asymmetry issue go away after breast implants removal?

Thanks for the question. As far as the pocket is concerned, once the implant is removed the pocket does not make as much difference to symmetry. However, most surgeons would agree that the capsule surrounding the implant (that makes up the boundaries of the pocket) should be removed at the time of implant removal to reduce the risk of fluid collection (seroma).

As far as the overall aesthetics of your breasts, removal of implants alone is not usually a good trade-off. Sometimes a breast lift may be performed with the removal, or the implants could be replaced and the pocket tightened up (capsulorrhaphy), either of which would likely make a much better appearance than simple removal.

Everything I Could Have Asked for and More!

Dr.McDaniel and his staff are so amazing! I came from Helena and it was so worth the drive visiting his office. I had two previous surgeries that I was not happy with and within 2 minutes of looking at me; Dr. McDaniel was able to tell me all the things he could fix. Visiting his office felt much different from the previous surgeon’s. I felt welcomed and listened to. I booked my surgery the same day as my consultation and the day of surgery was so easy and seamless. Recovery went great and I’m ecstatic about my results. 425cc Motiva Journey

I had my surgery 2 days ago with 425cc Demi motiva round implants under the muscle. Everything seems to be as expected so far, and I think I’m going to be happy with the final result and size I chose. I’m 5’4” 126 lbs and BWD 12.6cm. they’re sitting very high on my chest right now and are feeling really tight. Pain hasn’t been horrible- first night was the worst with 8/10 pain and discomfort but hasn’t been that high since. I slept a lot better last night. I attempted to shower but got really nauseous and almost fainted when my bf helped get the compression bra off and clean my sharpie off- so I just sat down and he wiped me with a wet cloth. Don’t think a shower is realistic for a couple more days. I didn’t expect the amount of back pain and aches I’m having from slouching and sleeping upright. I am constipated but personally didn’t get the swollen stomach as some do, just get really bad shakes sometimes if I walk or stand too long. I’ll post my updates here and hopefully it’s helpful to someone. Updated on 19 May 2025: Rough day. Took off bra for shower and nearly fainted from pain. Tried to shower day 1 post op but got extremely nauseous so I had my bf just wipe me with a warm cloth. A lot of swelling and very hard to sit up on my own or get in bed on my own. Updated on 19 May 2025: Lots of swelling still this day, maybe felt slightly lower down from my collarbones. They almost seemed small to me here, but I know they will change and overall I feel really confident in the size I decided on. I’m also happy they’re looking symmetrical for the most part and my incision hasn’t bothered me. Updated on 19 May 2025: Lots of swelling still this day, maybe felt slightly lower down from my collarbones. Really curious and anxious how they will drop and hoping for a successful healing journey. Very sore and tight, very tired and tough to get around. Applying lots of bio oil. Surgical bra is pretty tight and not comfortable but I’m trying to trust my surgeon and not wear one recommended from other places online. Updated on 23 May 2025: Recovery has been hard mentally. Up until today (9th day) I had very little ability to do anything and arm movement is still very limited. It’s hard to need help with everything and I think the pain meds affected my mood and emotions. Overall I’m really happy with the size and how I think they’re going to look. I’m just super anxious about them dropping and settling properly and they’re hard to look at right now because they’re quite squarish and the right one is sitting a little bit higher. I’m trying to put that out of my mind but it is unsettling. Overall just trying to manage my anxiety about everything and just be patient. Pain has been minimal the last couple days, especially today. I walked over a mile and kept my heart rate below 103 so I’m hoping I didn’t overdo it. I just really needed to get out of the house and move around because of the post op blues as they call it. Finally feeling more like myself today, still can’t wash my own hair or brush it which has been massively annoying as my bf is confused by all of this and despite doing his best, can’t even manage to put my hair in a bun. Ugh. But it’s getting better each day and I have a post op appt in 6 days- really hoping for positive feedback on how things are looking. Updated on 11 Jun 2025: starting to round, drop, and look a lot more normal. I go back and forth between liking them and wondering if I made a huge mistake. They look very big to me still and I really hope they won't look this big when they are done settling. But overall very glad they are starting to look more round! Will probably feel better about them once I can workout again and feel good in my skin. You can see in 3 week photo there's still quite a big of swelling on top and sides. It's still nearly impossible to shave under my arms.

Implant Upgrade (371cc to 470cc) - Already Under Muscle and Lesion Removal

After much searching, I set up a consultation with Dr. Jarred McDaniel. His office does charge a consultation fee, but it goes towards your surgery balance when you book a surgical date. My situation: Originally had 371 Natrelle silicone implants placed in October 2013. Never had any issues with my implants. I was discovered to have a 7cm lesion on my right breast that was benign but growing bigger to the point where it needed to be removed and the growth was plainly visible when wearing a bra/bikini. All this to say, I was searching for a plastic surgeon who worked beyond elective surgery and tackled physical appearance problems born from body malformations (like myself), accident, and injury. The consultation was thorough and Dr. McDaniel used the latest technology to capture my current stats and show my before 3D pictures on the (near) lifesize screen. I also viewed (potential) after models. We discussed everything. He gave me options as far as the game plan during surgery and the pros and cons of each. I was also encouraged to be in touch with ANY question. I would regret not mentioning Kayla, RN and Kate, Office Staff of Gallatin Plastic Surgery. I enjoyed meeting Kate at my consultation. You can tell she has years of customer service experience, as she is naturally warm, friendly, and smart about all the nuts and bolts of the office. Kayla, is one of those whipsmart ladies who has ALL the medical knowledge, but sprinkles in phrases like "that iv went in beautifully " and "you are doing wonderfully, Rosemary." to keep you feeling safe, confident, and excited about your surgery instead of fearful. Onto surgery day! Surgery was scheduled for 7a.m at a private surgical suite. I went this route before, preferring the more personal environment and avoiding the hospital. The suite was immaculately clean, sterile, and the operating room reminded me of Grey's Anatomy. I had the anesthesiologist chatting with me about medical "shop talk" and personal antidotes until I took a 2 hour nap. I was nervous, and that I was expecting. I woke up feeling sleepy and was discharged with thorough instructions until my follow-up appt. What a relief! For after care, I am taking Tylenol and Advil scheduled around the clock and pain medication as necessary. I CAN'T WAIT TO GET A PEEK AND SHARE PHOTOS WITH YOU, LADIES!! Updated on 18 Mar 2025: Here are my before photos! You can likely notice the asymmetry as the left inplant had drifted down and out and the right breast is where the 7cm benign growth was located. I had the pocket for the implant tightened on the left side only. On the right side, where the growth was removed, Dr. McDaniel was able to go in through the inframmery incision and was able to only make one incision instead if two. This I am thrilled about! Updated on 18 Mar 2025: Here are my 8 week post-op photos! The left breast continues to drop and fluff. I am 100% satisfies with my results. I was given complimentary scar treatment cream and a discount on scar laser treatments in the future, though I don't think I will need them! I plan.on posting more photos after my final visit with Dr. McDaniel at 6 months post-op. Updated on 20 Mar 2025: It looks like my 8 week photos did not upload with my update. Let me try again! I have started using the scar cream and like how quickly it absorbs. It's also wonderful having that huge growth out of my body on the right breast side. Dr. McDaniel is phenomenal! He consulted with the ER when my son was brought in for an eye injury/tripod fracture. He supported the ER with recommendations in order for us to go home safely. We followed up with him two days after our ER visit, and he was very helpful. He reviewed the CAT scan results with us, and he made our 13 year old son feel very comfortable.
