Amazing and gentle dermatologist that knows what he’s doing! Great bedside manners and my lips turned out amazing despite my pickiness. Not just that but he is super knowledgeable about skin and other skin related issues. I will be back again to do more treatments. So glad I found a doctor I can trust.
It looks to me like you have rolling and boxcar scars caused by atrophy of the dermis. You would most definitely benefit from nokor needle subcision. I would have recommended subcision first in your case, before lasers/RF/chemical peels.
It takes months for the effects of Sculptra to show, so yes your scars may continue to improve as more collagen is formed in your skin.
I think you would benefit greatly from taylor liberator subcision which is a fantastic way to improve deep bound down scars, but also nokor needle subcision to improve the scars that are mainly composed of dermal atrophy (the more superficial ~1mm or less in depth). I've found that nokor needle can almost erase the depressions from dermal atrophy, although it is very challenging to do for rolling scars because the skin is not evenly depressed. It works great however for scars with sharp edges and relatively even depressions like the box car ones you have. Combining subcision with filler either biostimulating or fat will probably give you a better result too.
Based on your photos, I think nokor needle subcision is the perfect treatment for your scars. You may not need TCA cross. It's a shame that many people treat these type of scars with lasers and microneedling first when they would benefit most from subcision.
Yes, subcision can be done on the entire face and is the most effective treatment for rolling scars. I've found nokor needle subcision to be superior to cannula based subcision techniques, and I think would work well in your case.
