Thank you for your question. I agree with the previous answers in general. The strength of the repair is very important, but the strength of your native tissues and their ability to heal in position is also crucial, as is the muscle position and strength. I frequently utilize dissolving meshes like galaflex to help re-inforce repairs like this, because so many variables are out of my hands. These reinforcement structures can help protect the repair while healing. Hopefully you will not have any issues. good luck!

While your photos do suggest you would benefit from some facial rejuvenation, especially in the jowl region and under the neck, further evaluation would be necessary before an appropriate medical opinion could be rendered. Thank you for your question. You have a very clear discrepancy in size and shape of your breasts, which is not that uncommonly seen. You will likely need a mastopexy (lift) to the left and implants placed to both breasts for the ideal result. While you may be a "c" on the left side, once the skin envelope is reduced to better equal the volume you actually have, you will likely be a cup size smaller, and thus closer in pre operative size than you think. (Albeit still different enough to require differential implants) Fat grafting may also be a valuable adjunct to your surgery to help create as much symmetry as possible. Make sure you visit a board-certified surgeon who does complex aesthetic breast surgery routinely. Good luck!

Thank you for your question. The recovery from breast augmentation is typically fairly rapid. Within our practice, we encourage patients to resume normal activity essentially immediately after surgery. Exercise is OK at 2 weeks, such as riding a stationary bike, running or intense cardio is allowed at 4 weeks, and heavy lifting or repetitive activity (such as scrubbing and sweeping for long periods of time) is best withheld until 6 weeks post op. Thank you for your question. You still have residual swelling to your lower abdomen, which is not uncommon. However, if you have an area of discrete swelling or firmness there may be a fluid collection that needs drainage. Otherwise it may just take some additional time. Thank you for your question. Your post operative result does indeed take 12-18 months for the final result. Further, what feels like very firm tissue (bone) may still be swelling of the soft tissue of the nose. It's simply too soon to tell (both hump and width). It appears you will have a very nice result in the tip region even at this early post operative period. If the dorsum continues to stay elevated over the next few months, that would be an early indication that there may be some residual hump. I recommend you stay in contact with your surgeon, and if at 3-6 months you are still concerned about persistent fullness, you may want to entertain a small possible revision to the dorsum. For now though, just give it some time! Good luck.

Thank you for your question. You are still very early in the process, so I would not be too concerned about asymmetry at this time. Also remember, some asymmetry is very common both before and after any breast surgery. I recommend you follow your surgeon's advice regarding any post operative exercises and re-evaluate your result in a few months. Good luck.

Any plastic surgeon or facial plastic surgeon who specializes in rhinoplasty should be well trained to perform a turbinate reduction.  There are multiple ways to perform this procedure and the one best suited to you will depend on your particular anatomy.  I recommend you visit with a board-certified plastic surgeon or Facial plastic surgeon with whom you are comfortable to help you make the best informed decision. Thank you for your question. No one factor should likely make your decision on Surgeon choice. That said, if it is something you are strongly interested in, then you should see if your surgeon will consider it in your case. The cost is approximately $400 (I use in almost all abdominoplasty cases) and I find it greatly helps my patients in their recovery. There are other options for post operative analgesia,  but nothing as effective in my opinion. Good luck!

 Great question. I would recommend a minimum of six weeks in between surgeries and ideally three months. This is simply to allow your body time to recover from surgery as rhinoplasty and buttock augmentation could theoretically be done at the same time as long as Operative time is not excessive. Good luck!

Breast augmentation, upper blepharoplasty & CO2 laser to my face

Dr. Unger and his staff are incredible and very accommodating! At the first consult I was skeptical of having anything done, but I knew Dr. Unger was the surgeon for the job. He has a great bedside manner and is confident. I always felt like I was in good hands. I decided to have a breast augmentation, upper blepharoplasty, and CO2 laser to my face. I am terrified of surgery and needles and they made me feel very comfortable every step of the way. I showed Dr. Unger pictures of results that I hoped for and told him that I trusted his judgement and he could not have done a better job or chose a better size for my body. I healed nicely and was even on the lake on day 5 and 6 after surgery! I am now 2 weeks post surgery and I’m happy with my breasts, my skin is glowing (even getting compliments), and my eyes look more awake and open. Dr. Unger is simply the best!

I feel more confident in my body today than I ever have in my life!

Consult- I searched the internet far and wide before choosing to go to Dr. Unger for a consult for a Mommy Makeover. My 40th is fast approaching and this would be my version of a midlife red convertible sports car. After meeting with him for a consult I knew that he understood what I was asking for, what was most important to me and also the things I was nervous about. The price tag was steep, but if you’re going to hack your body up, bargain shopping is not the move. Pay for skill. Day Of— On the day of surgery I was pretty nervous. His nurse anesthetist was AMAZING. Not only had we chatted prior about my concerns and health history, but she was fabulous the day of and kept me calm as did his OR nurse. Dr Unger also met with me to review the plan and confirmed that he understood what I wanted and what was important to me. Once I was out of the OR I was actually surprisingly awake. I had a gingerale and once all was stable, my husband drove me home and I was wide awake— and nausea free! Thank you, anesthesia goddess! Once home, I thought I’d sleep. I did not. I was awake watching shows, but the discomfort did start to set in as the day wore on. I’m not a fan of pain meds, but I needed them. I did 2 c sections with no post op pain meds, but plastics aren’t for wussies. The recovery process required meds. Take the meds. They give them because you WILL need them. Recovery—the drains were very annoying to me, but I knew their purpose and got through those. First one was pulled at Day 6 post op and the second on Day 8. That was a glorious day! I used a walker for the first week after surgery and it was THE BEST THING. I was off the pain meds after probably 4 days and didn’t use the full RX, but I did need them and that surprised me bc I feel like I’m pretty tough. The first week was weird, it’s all a bit blurry and I definitely felt a post anesthesia fog. Pain-wise it was a struggle to get around and stay comfortable for the first two weeks. By week 3 I felt so much better and by week 4 I was doing a lot more than I could previously. My boobs really didn’t bother me much at all, almost all of my discomfort was related to the tummy tuck/lipo. Post-Op: I’m 6 weeks out today and my results are FANTASTIC! My stomach is FLAT. It’s never been flat in my life and that’s so amazing to feel and supposedly it’ll look even better over the next few weeks! My boobs look like what I had before these kids ruined my bod and are exactly what I asked for! I was a size 2 at my consult (after 15-20lbs of weight loss) and I’m still a size 2 but the waist is a bit looser. I’m writing this because I feel bad that I’m not brave enough to consent to him posting my pics to his website, but he did amazing work and I feel more confident in my body today than I ever have in my life! Thank you, Dr Unger! I’ll see you for a facelift during my next midlife crisis!

One day post-breast lift - so far I am thrilled with my results

After researching breast lifts for the past decade, I finally went through with the procedure after finding Dr. Jacob Unger here in Nashville. My breasts have always sat lower on my long torso and years of going off and on birth control and weight fluctuations had caused a major loss in density. My breasts were deflated, hollow, stretch-marked, and sagging and I hated how they looked in (and out of) bras and swimsuits. I visited a number of doctors but they all pressured me about getting implants - which I was adamant I did not want. One doctor told me my breasts would be "saggy" without an implant and another refused to entertain that I would not be having children after my surgery. I felt unheard and condescended to. During my consultation with Dr. Unger I immediately knew I had found the right surgeon. He was confident that I could still maintain volume without an implant and said that I would have "modelesque" breasts to fit my tall and thin frame. A much better word choice than "saggy". Not everyone wants large breasts with implants and he gets it! The staff at Dr. Unger's office have been so wonderful during all visits. From his nursing team to front desk reception to the billing staff - it's been a pleasure interacting with everyone. I can honestly say that I was not one bit nervous before my surgery, I had that much confidence that I was in the best hands. I've spent years scrolling through before and after photos on Real Self and it was an invaluable tool for me to see the reality of what to expect with a breast lift. I'd like to share my before and after journey on this platform as I continue to heal. I hope it helps someone make their own informed decision. I'm just 24 hours since my breast lift but I will continue to post updates throughout the process. So far I am thrilled with my results and can't wait for my breasts to heal and settle into place. No regrets! Updated on 2 Jun 2022: I am one week into recovery and just had my first follow-up visit with Dr. Unger. Everything is healing as it should and swelling is slowly diminishing. Bruising has increased but wearing a tight sports bra 24/7 helps me feel secure. I stopped pain medication after day 4 and finish up the last of my antibiotics today. Overall the hardest part about recovery has been not lifting anything over 10 lbs (everything is over 10 lbs lol!) and not exercising. I’ve found my hunger levels have increased as well. My steri strips are still tightly in place and I put bacitracin on the “T zone” twice daily where there is the greatest pressure from the incisions pulling in three directions. The doctor’s office gave me scar gel to start using in 2 weeks when my steri strips come off. I’m anxious to see what my areolas and incisions look like under all that tape. I am still very pleased with my results and looking forward to seeing the final breast size and shape as they continue to heal. I will post another update when the strips come off. Updated on 10 Jun 2022: I planned to wait until next week to post, when my tape comes off, but bruising is almost gone and I wanted to share. I bought my first backless dress (ever!) and have gone down a size in tops and dresses - all thanks to the lift. I went for my first exercise walk at two weeks but kept it short at one mile. My lower incisions are still tender but I plan to walk at least a mile every day until I’m cleared for more intense cardio at 4 weeks. I experienced the “zingers” 1.5 weeks into recovery which was quite a shock- literally! A google search explained this was normal to feel as nerves that were stretched during surgery loosen and repair themselves. Thankfully it only lasted 48 hours because it was painful and a strange feeling. It is a sign of healing though so I’m happy that my nerves are healthy and functioning as they should. While my breasts are much less sore, I have developed muscle pain in my back. Another google search described this as the result of “protective posturing” which is when people hunch their shoulders and back after surgery to instinctively protect themselves. I’ve absolutely been posturing like this and am now conscious about arching my back and sitting up straight. I also found some stretching exercises online that are recommended after breast surgery and will be adding those to my daily routine. Updated on 16 Jun 2022: My third week of recovery was relatively uneventful and I’m feeling much more secure and less fragile. Bruising has completely cleared and swelling continues to slowly decrease. I’ve been walking for exercise and doing arm movements and stretches to increase my range of motion. My steri strips had not fallen off naturally so I was instructed by my doctor to remove them at 3 weeks. I got my first look at my incision scars and areolas and overall I’m really pleased. The incision lines are much finer than I expected and I think they will look great once fully healed and flattened. I begin using twice daily scar cream today and will continue that for the next 6+ months as my scars soften and lighten. There are a few sutures that are poking out but that’s normal and if my body does not fully absorb them then my doctor can remove at a future visit. In one week I can begin running and more strenuous cardio and I’m looking forward to that milestone. I will post my one month photos next week. I’m still very satisfied with my results and will continue to share my recovery journey. Updated on 26 Jun 2022: One month after surgery and all is going well. Swelling continues to slowly decrease and incisions are healing. There's still some soreness around the incisions and I've pulled or trimmed a few stitches that were pushing out of my skin. Overall I feel much more secure and less fragile than the first few weeks. My range of arm motion is greater as well. I have my second follow-up appointment in two weeks and should be cleared for all exercise and to wear regular bras again. I'm currently measuring a 30DD and was wearing an ill-fitting 34DD before surgery, so a shopping trip is much needed. I will update again as the scars continue to soften and lighten. Updated on 24 Jul 2022: It’s been two months since my breast lift and everything is going fabulously! I’m back to full exercise activity lifting weights and don’t feel any pain or tightness at my incisions. Scar lines are slowly flattening and fading and I apply scar gel twice per day. At 6 weeks I was able to wear underwire bras and I continue to sleep in wireless, front-zip bras to help speed up scar healing and (hopefully) fight gravity. Swelling greatly reduced around the 5-6 week mark and my surgeon said that my breasts will reach their final shape and position by 6 months. I had my second follow-up with Dr. Unger and he was very pleased with my results. His nurse took photos for my file and there were lots of hugs shared. I went shopping for new bras and now wear a 32DD, or 30DDD, depending on the bra. Sizes vary between brands but I wear Natori, for reference. I’ve gone out braless in public (something I’ve never done before) and it was an amazing feeling. The range of dresses, swimsuits, and tops I can wear now is limitless and I finally feel confident in my own skin. My only regret is that I didn’t do this years ago!

Amazed at how little bruising and swelling I had

Dr. Unger and his entire team are all amazing! He has the best bedside manner of any physician I have ever encountered in my 61 years. I had Blepharoplasty (eyelid surgery) and was amazed at how little bruising and swelling I had in comparison to many of my friends that had the same procedure from other plastic surgeons. I did follow all the instructions given to me for post-op to minimize those issues since I had it done so close to the holidays. Jonathan was fabulous to work with for scheduling since I requested moving up my appointment if they had any cancellations. He is so friendly, professional and a hoot. The surgery itself was a breeze and took less than an hour. Karen, my OR nurse, was fabulous too. She is so kind, caring and made me feel at ease even before administering my Valium. Every member of his team was so friendly and professional and I just can’t say that enough. But, most importantly, I am most impressed with Dr. Unger and his fabulous work. (He is not bad to look at either!) I choose him because he had done such a great job on my sister’s breast reduction. Her scarring was so minimal compared to mine which was performed by a different but highly recommended plastic surgeon in Nashville. In the photos I posted, the first photo is the day after surgery. The last photo is about a week later and was as bad as my bruising ever got. Using baggies of crushed ice helped me more than any other method to really get into the eye sockets.

Dr Unger is very talented and knowledgeable about new technologies.
