This appears to be a hypertrophic scar. The difference revolves around the extent of scarring outside of the original incision. In this case the vertical component of your abdominoplasty scar appears to have widened but remains along the original incision. Steroid treatment is certainly a good starting place but if this has been attempted and the result is not satisfactory to you then surgical revision may be your next step.
