It seems you would be an excellent candidate for a tummy tuck. You may have to speak with your plastic surgeon again about what you can expect as far as loose skin. If the tummy tuck is more important than a BBL, you may have to do surgery in 2 sessions. This is a subject you should probably discuss with your plastic surgeon before going further with plans.

It is difficult to tell just from one photograph, but it seems as if you have a very significant loose area of your abdominal muscles. This is much more easy to diagnose with and in person exam. You could visit a plastic surgeon and they could give you a good idea of what you could expect further surgery.

I like using sizers with a sports bra with my patients to help them choose a certain look. I think it is also very helpful just to look at the implants themselves… A 350 cc implant might not have the dimensions or volume you are looking for. It seems you are very educated on all of the particulars that plastic surgeons consider. Do not hesitate to return to your plastic surgeon to look at sizers and discuss your expectations again.

It is difficult to understand what might be best for you. I am not sure what your starting point was prior to surgery. Considering a fleur-de-lis is a big deal considering the scar… Even when some patients just stand in front of the mirror. Other things can be considered like liposuction and flap advancement, but probably would not want to consider this for another year or so. Remember that when you bend over, you will always have rolls of the abdomen tissue and you should judge the overall look only when you are standing up straight.

From your photographs, this certainly seems like capsular contracture bilaterally. An examination by your plastic surgeon would probably produce a definite diagnosis. Capsular contracture is very frustrating for everybody, but in most cases, it can be corrected and the patient left with an excellent result.

You seem to have the real issues laid out, its now a matter of deciding about upper pole fullness and of course nipple position and scars. It seems like your previous scar tissue from your C-section and bladder surgery would not be a concern as far as proceeding with a tummy tuck. The internal scarring from the previous surgery is deep to the abdominal wall muscles and therefore not a problem.

A good approach is to just imagine that without a breast lift the nipples stay in about the same position. If you do not mind this, then you could certainly proceed with just augmentation. It would be common in a case like this is that the breast sagging continues over the years and you may want to consider a breast lift in the future.
