Hi,

Firstly, it is best to tackle more advanced acne and acne scarring such as this with a range of treatments spread over frequent sessions.

I can see that there is some active acne present, and this will need to be dealt with prior to treating the scarring. I would prescribe manual extractions followed by chemical or herbal peeling coupled with ClearSkin laser in addition to good homecare products. This is to ensure the active acne is thoroughly dealt with and to reduce the risk of further scarring.

Once the active acne is dealt with we can focus directly on the scarring. Initially I would start with several sessions of ablative skin resurfacing laser to manage the more shallow scarring and even out the skin. 

I would then review the result and treat any deeper scarring left behind with scar subcision and PRFM. This involves the surgical untethering of the scar to the underlying tissue using a hypodermic needle, followed by injecting Platelet Rich Fibrin Matrix (PRFM - from the client's blood) into this space. The Platelets aggregate and stimulate the healing process via the release of various growth factors. 

The end result is smoother and more even skin.

The stretch marks will not worsen initially and will gradually improve with Carboxytherapy. You may notice that the area treated will be swollen and redder than usual for a maximum of a few hours post-treatment.

Carboxytherapy works by increasing circulation, oxygenation, and collagen production in the target tissue. The stretch marks will become softer and closer to skin tone of the original tissue, however it is highly unlikely they will disappear completely. A good result with Carboxytherapy is one where the original stretch marks are barely noticeable.

Carboxytherapy as a standalone procedure is a long term solution. I would expect clients to require 10 treatments, at a frequency of twice per week, as a minimum for a good noticeable result. In my clinic I combine Carboxytherapy with several other treatments, an example being LPG Endermologie. The LPG works similarly to Carboxytherapy (stimulating an increase in collagen production) but via mechanical massage. I find the multiplicative effect of both treatments results in not only a faster result, but a superior result.

For severe stretch marks I would use Plasma Pen therapy. This treatment is more invasive however gives extraordinary results with as little as one treatment. This treatment is performed under topical anaesthetic and does have a downtime of 10 days with outstanding results after this period. I would recommend Plasma Pen over Carboxytherapy in severe cases. LED therapy within 2-3 weeks of surgery has been proven to reduce healing time; as well as reducing swelling, pain, and redness. This works as red and NIR light absorbed from the LED triggers cellular responses that lead to improved tissue repair and regeneration.

I use LED therapy in my clinic and find that the number of treatments required varies between individuals and conditions. There is no real "limit" for frequency of LED treatments, but in practice it is best for your healthcare professional to monitor your therapeutic response session to session to determine optimal frequency for you.

Standard use of Profhilo calls for injecting 0.2ml in each of the 10 points (5 on each side) each session. This totals to 4ml of Profhilo used over 2 sessions. 

I don't see the benefit of injecting less than the standard amount as you risk losing some of the results of the treatment with no potential gain, other than a slightly cheaper bill. It is never worth sacrificing quality of treatment for a lower price.

It may be in your best interests to get a secondary opinion in a consultation with another trained professional if you are not feeling 100% comfortable.

This is called "Sebaceous hyperplasia" which  is a very common condition that presents as small bumps on the skin. This is caused by an enlargement of small sebaceous glands in the skin. Sun exposure increase the risk of getting them.

With ageing, the androgen hormones decrease. This slows down the sebaceous gland activity. And it's not just oil production. The natural cell turnover rate within the sebaceous glands slows down as well. The cells back up within the gland, causing that overabundance and enlargement of the gland.

There also seems to be a genetic link. It is not contagious.

Treatment involves:

-Retinol, Azelic acid to increase cell turn over

-Exfoliation 

-Laser

-Plasmapen fibroblast

-Cryotherapy/electrolysis/cryotherapy.

-Sun protection is a must.

If you have any concern about the nature of them, It is always important to be seen by a physician to get a proper diagnosis. Because sebaceous hyperplasia looks incredibly similar to the much more serious basal cell carcinoma, you should have your bump(s) looked at by a physician to know for certain what you're dealing with.( The good news in your case is that they been there for long time and no change occurred to them) 

Sebaceous hyperplasia is completely harmless. 

Many people with sebaceous hyperplasia choose not to treat it.

Terrible Scars - do not repeat my mistakes

24 May 2022 I came to dr.Hala’s cabinet in Fulham to eliminate one pigment spot on my cheek and another, under my left eye (not visible, but she said I had to do it also) She chooses a definitely wrong method - plasma-pen, with which she fired my skin until I was feeling the burnt meat smell. It was so painful, but she persuaded me that everything is going right. When I was doing laser treatment before, it was never happening. On the first picture you can see before, then after 2 weeks, when both spots are starting to be extremely red (I was using all the skin calming treatment) and how it is looking now. It’s much more, then I had before. I asked my regular specialist from Barcelona, and after examining we discovered really huge and deep BURN which is also growing now. Now I need an expensive professional treatment to heal the spots cause now I’ve got two holes on my face with another structure. Part of my skin derma is totally destroyed. So if you will have enough courage to go to such a “specialist” you also need need to think about money, cause she will tell you the price basing on how much can you pay her. She will check how you are dressed, looking and then she will tell you the amount. Out of the price list, of course. So I paid 100£ for scars on my face, and when I came to her to solve this problem by treatment or at least to get my money back, she kicked me out in a very, very bad and rude manner. I stopped to be a “sweetheart” in a second, just because I asked to do something, I was behaving myself very calm and patient, I didn’t expect this kind of “treatment” at all. Girls, do not repeat my mistakes, you’ll be so disappointed cause she will leave you alone with skin problems, like I’m having now. Check several times before going.
