The most likely reason for this is an uneven relation on the way the upper and lower crowns are biting (bad occlusion)... the back teeth have cusps that need to be correctly related to their counterparts [top or bottom], if not, it can cause tissue [lips, cheek] biting. Hope this helps.

An X-ray needs to be done to determine if there is any damage on the roots or the surrounding tissues, if there is no damage on the inside and the extent of the visible cavity is not that much, a filling could be placed, but Its important to let you know beforehand that its a borderline case, and these minor variables could turn the treatment balance to one side or the other. There should be no pain or at least almost no pain after 10-12 days... I would try giving it an extra 3-5 days to see if the pain remains an to take an x-ray... if the pain is still there after this time, it's not a good symptom and the implant will be probably failing. Hope this helps.

The space needs to be measured in order to determine whether a small implant should be placed or just place a bridge... placing a disc to create space is not a viable option and also the bone loss has to assessed in order to reestablish the adequate bone height if an implant could be placed.
