I was referred to Dr. Gladstone by my ophthalmologist and I've been very pleased with my experience. The staff was great and supportive in working with my insurance and the results were great! I had been told I looked angry and hadn't realized how diminished my field of vision had become over time. The surgery went as planned and my recovery was quick and non eventful. I wanted a procedure performed to diminish the signs of aging around my eyes. A lower eye lid laser procedure was performed under local anesthetic in the outpatient surgical suite adjacent to Dr. Gladstone's ophthalmology practice. I was very impressed with the care and attention given by Dr. Gladstone and all the medical professionals associated with the process. The patient instructions were clear and concise; Dr. Gladstone was available if problems arose. I felt confident and comfortable that my needs were addressed.
In January Dr. Gladstone performed Blepharoplasty on the upper eyelids on both eyes. I had noticed over the past year and a half that my field of vision had diminished and I was suffering from extreme eye fatigue when I read. Dr. Gladstone provided excellent detailed information during the consultation prior to the procedure and answered all of my questions. I was scheduled for the procedure within 2 weeks of the consultation. After the procedure, I was given detailed information about post-procedural care that I followed carefully. To my surprise I had very little pain and I am delighted at the results. I have seen a marked improvement in my ability to read for longer periods of time and have a much wider field of vision. My eyes look great. The incisions are unnoticeable. I can't say enough about the professionalism, efficiency and care that Dr. Gladstone and his staff provided. I highly recommend Dr. Gladstone.

Without examing you I cannot say for sure, but from your photograph it appears that there may be a little residual skin which could be removed. However, you don't want a surgeon who is too aggressive with skin removal because this can lead to serious eye complications, such as dry eye and corneal exposure. If you are conservative to start, you can always go back and remove a little more skin. Doing a ptosis repair (which involves the muscle) would not help what you are seeing with the eye closed. As far as fat, again I would need to exam you to be sure, but it does not appear from your photograph to be a big issue for you. In your case I might also discuss the option of a browlift with your physician. This is because you are expressing a desire to have “open eyelids”. Removing skin alone will often not provide any visible upper eyelid the way raising the forehead can. I use an endoscopic technique so that small incisions can be hidden in the hairline. I have seen very good results in patients with a desire to have visible upper eyelid, which is probably what you are envisioning when you say ‘open eyelid’.
I am guessing that you had 1 vial (rather than 1 unit). Although it is generally “safe” to have more Botox right now, it is probably unnecessary at this time if your underarm sweating has not returned yet. There are a couple of considerations: 1) Clinically it is best to use the smallest dose possible at the longest interval that will control your symptoms. This is because in general you don’t want to use more drug than you need and also because people can develop some resistance to Botox therapy. 2) You don’t want to pay more frequently than is needed for this therapy and if it is covered by your insurance you will want to check your policy because many plans will only pay for this therapy every 90 days. If Botox was effective for you it will likely continue to be effective, but I would recommend making a return visit once your underarm sweating begins to return.
With twilight sedation you should have no trouble with pain from the injection of local anesthetic. I recommend sedation for almost all of my upper eyelid blepharoplasty patients for a more comfortable experience, particularly if they are nervous about surgery or pain.
This is most likely temporary puffiness caused by lack of sleep, however if it persists or worsens you should see your eye doctor to make sure that it is nothing more serious. Watching your salt intake and using a cold washrag may help reduce this swelling, but the most effective remedy would be trying to get more sleep. However, particularly given your young age, unfortunately it is likely that the bags in your lower eyelids will eventually return and gradually worsen over time. Surgery removes the fat that is present, but cannot prevent more fat from pushing its way forward into the lower lids (we call this herniated orbital fat). Heredity often plays a strong role so if your parents and/or other relatives developed this condition at a young age it is not that uncommon to see this problem begin in the late 30’s or 40’s.
Orbital decompression surgery is typically performed when the muscles around the eye become swollen and may compress the optic nerve. This is often related to Graves’ disease (a thyroid disorder). The surgery creates space for the swollen muscles to relieve pressure and prevent damage to the optic nerve. This is a fairly major surgery and I would not usually recommend it solely for cosmetic reasons, particularly before more conservative treatment has been tried. From your photos, it appears as though you might get nice cosmetic improvement from performing a lower eyelid retraction repair surgery. This would raise the lower lids and eliminate the white of the eye that is now visible below your iris (colored part of the eye), which causes the eyes to look protruded. This is a less invasive surgical option that would only take about 45 minutes total to perform on both lower eyelids. If you have not already had one, I would also recommend getting a thyroid work-up by your internist and/or an eye doctor with expertise in Graves’ disease. Because exophthalmos (protruding of the eyes) is commonly associated with this disorder, it is good to have this checked in case any other medical treatment is warranted.
