Swelling and "lumpiness" 3 weeks out from surgery are normal post-operative findings. The deeper tissues below the skin take time to heal and settle. Regarding the "large lump", it is difficult to appreciate based on your photos. In some instances there can be collections of old blood/fluid. If large enough they can manifest as you are describing. I would recommend you see a board certified Facial Plastic Surgeon/Plastic Surgeon for evaluation. In some instances these can result in permanent swelling and occasionally lead to infection.

Given your age and description I would think that cervical (neck) liposuction with or without Thermi Tight would be a good option. The liposuction would address any residual fat and the Thermi Tight would be meant to improve skin laxity. Additional photos would be helpful. I hope this helps and have a great week.

Based on your photos I think you are a good candidate for cervical liposuction with a facelift. You could also add a neck lift to refine the central part of the neck as well. If budget is a concern then I think you would get the most improvement with lipo and a facelift. Some patients can benefit from liposuction alone but achieving a good result is dependent upon skin elasticity. Without actually examining you I am unable to make that determination. Another factor that needs to be considered is your skeletal anatomy. If your hyoid and thyroid cartilage sit high and back then you are more likely to get a favorable result compared to someone who has a low lying hyoid/thyroid.

Treating keloids will require multiple approaches. If the keloid is pedunculated you could treat with surgery to remove but I would also recommend treatment with Kenalog injections as well. In addition, you may want to also consider silicon sheeting. I would recommend a consultation with a facial plastic surgeon in your area to give you a good treatment plan.

The short answer is yes you can have a septoplasty after a rhinoplasty. I would recommend that you wait 2 months following the rhinoplasty to allow swelling to decrease. By no means will your cosmetic changes be settled by this point. However, addressing the septum will typically improve airflow through the nose and should not disrupt the appearance of the nose. Providing additional photos of your nose would be helpful. The current photo is too far away to get a good impression of the appearance.

Facial resurfacing with the Er:YAG laser or CO2 laser are 2 good options to get you immediate improvement. You must realize that there will be several days of downtime while you are caring for your skin. The post operative period is not painful but will require frequent application of ointment during the first several days. This procedure can only be done by a board certified facial plastic surgeon or BC plastic surgeon. The jowl liposuction will have swelling that will take 4-6 weeks to settle. However, realize the tissues will be settling out over several months. The Thermi tight will take months to fully settle. This does not mean that you will be swollen during this time period. Most patients will experience some "tingling" in the area that was treated.

You can use Radiesse to augment the nose but be careful where it is injected and who injects the filler. You can place it in the radix (root of the nose) and in the tip region carefully. These areas are thick skinned. Areas of thin skin, ie: dorsum, sides of nose, can be challenging due to the lack of subcutaneous tissue. I would recommend using an HA (hyaluronic filler first), it is more malleable and able to be dissolved if you don't like the result. The area in your picture is difficult to clearly see but I would probably try a HA first.

The short answer is no but I will tell you that this would depend upon how much of the turbinate was previously resected. You do NOT want to too much tissue removed. This could lead to a sensation of no airflow through the nose (empty nose syndrome). I would strongly advise that you get more than one opinion and you should also undergo nasal endoscopy and possibly a CT scan. You need to rule out other possibilities for nasal obstruction (septal deviation, enlarged middle turbinate, nasal polyps and allergy).

I would recommend to massage the neck about 3 X a day (approximately 2-3 min per session) to help soften any contraction/scar tissue associated with the surgery. The scar tissue is still healing at 3 months out and should soften somewhat over the next few months. Another option to consider is kenalog (steroid) injections. It would also be helpful to see some before photos. I would not do any kind of revision until you are at least 6 months out. I know it is difficult but please give it some time and continue to follow up with your surgeon.

I have obstructive sleep apnea due to having deviated septum. I had the full Imagelift with the Laser Resurfacing. My skin showed wrinkles on my cheeks, the skin under my chin was starting to droop and my jawline was sagging.
