It depends on whether it causes any issues or symptoms. Those of concern would be (1.) a symptomatic fistula with leak of fluid or food into your nose, and (2.) changes in speech. Neither of these are likely to be "dangerous", but you would be advised to see a plastic surgeon who does cleft surgery for a proper assessment.

Rupture is a silent event without symptoms or signs. With 27 year old silicone implants, and especially with the event and features you describe, you should prepare to have them removed/replaced, but they should be scanned beforehand to check for any other breast issues and to check the integrity of the implants.

The cleavage is the distance between your breasts which should be, as a guide, 3 cm. It looks ideal in you. Perhaps you mean upper pole fullness in which case this will become less with time as things descend and your breasts soften. In my practice, I usually advocate the support bra for 3 weeks post op after which you can be fitted for normal bras. You should of course check what your operating surgeon recommends. The only issue with regular bras may be the underwire which can irritate the scar and cause discomfort, but if this is not the case then feel free to wear any bra you like. Bandaging is likely to keep everything up and make your issue worse, so let that go. All the best. Hope it is of some use to you.

Can liposuction remove man boobs?

Yes, most cases can be done with lipo alone and sometimes even glandular tissue can be removed through the lipo cuts with a rongeur. For gynaecomastia lipo I use a special lipo cannula which has many more holes than usual and has a slight grater like effect that helps remove glandular tissue. Areolar or nipple reduction and severe or really large gynaecomastia may require more open surgery, but in my practice this rarely needed.

Yes, especially with an auto-augmentation where breast tissue from the lower pole is moved up underneath the breast to provide fullness in the upper part of the breast. Botti and I both have videos showing this procedure on youtube. But of course it depends on the bra - a good bra with an underwire is designed to lift the breasts too. Judging by your photos, you would benefit from mastopexy.

Fat transfer has been presented frequently by some high profile names at our meetings and has become a trendy procedure, but in my opinion falls short of fat transfer for a number of reasons:

1. it requires more dissection and therefore is more invasive. There is less risk of disrupting the support mechanisms of the lower lid.

2. there needs to be an exact balance between the amount of fat excess and fat deficiency in the tear trough and lower lid to adequately fill these areas. Often there is not enough and the lid remains empty to a degree. Primarily the midface and lower lid age with deflation and so filling is necessary and frequently there is inadequate fat in the lower lid bulges to properly fill even the lower lid and adjacent mid face.

3. fat transfer with fat grafts introduces stem cells which improves the skin quality. You don't get that with fat repositioning.

4. fat transfer with fat grafts provides a subtle lower lid lift which is desirable to counter the effects of aging.

5. fat transfer allows more control in contouring the lower lid.

Fat grafting of the lower lid needs to be done with subtlety and care, so I agree with the previous answer: look at plenty of before and after photos!

Paper tape helps to support the scar and will improve the quality of the final scar. The stimulus for scar tissue production is movement and paper tape helps reduce this movement locally at the scar, thus diminishing the amount of scar tissue formed. Paper tape should be used while the scar is active - red or raised - so for at least 6 months and maybe even longer if you can. It should be changed infrequently, i.e. only when necessary, to avoid damaging the underlying skin with repeated removal and reapplication of tape. I advise patients to use it for all scars from any procedure if possible.

Over the next couple of weeks your lower lids will improve, and as they do the post-op ectropion will resolve. Gentle upwards massage a few times a day and taping will hasten the process. The massage is a form of manual lymphatic drainage which will help with swelling and it will help with positioning too. From the photos it appears as if your left lower lid is a little more droopy than the right, but I would expect both to resolve without the need for further surgery.

You do have the typical deflation changes of facial aging and this is especially apparent in your lower lids which still appear hollow. This despite some post-op swelling. Some degree of lower lid lift can be achieved with fat grafting of the lower lid which can push the lid margin up. The fat grafting can improve the lower lid hollowness, tear trough and blend the lower lid cheek junction.

One of my colleagues above says you do have gynaecomastia (mild) and the other says you don't! That must be a bit confusing for you. While your chest looks pretty normal to me, if there is a small, firm mass or hardness under your nipple then that is likely to be gynaecomastia. One needs to look AND feel before one can make a diagnosis. If there is, and it bugs you, then you can get it surgically fixed (there is no other way). If it does not bug you, leave it alone. Training your pecs, as has been said, will enlarge your muscle, have no bearing on the breast tissue, but larger pecs can push any breast tissue forward and make it more visible.

Hey everyone, Ever since I have been planning for my BA a few years ago, I have been on this site non stop and it's finally time to post my review ! I had my BA done with Dr Dirk Lazarus, and I am absolutely in love with my results!! We settled on 300ccs demi profile and they look beautiful and natural. Much better than I expected to look. The whole team at the tranquillity clinic are absolutely AMAZING!! they really treat you so well, my experience was 5 stars for sure.  Regarding the actual surgery - I arrived at the clinic at 7 30, a bit nervous but mostly excited and sure to stay positive. Everyone made me feel really comfortable and soon enough it was time to hop on to the theatre table. The anaesthesia they gave me really just made me feel like I was falling asleep naturally, and it wasn't like other anaesthesia I've had where you can physically feel the chemicals coursing through your system as it slowly starts to work which felt absolutely terrifying !  When I woke up I was very hazy, it felt like I had awoken from the best sleep ever haha.... I was in basically no pain when I awoke, and immediately wanted to take a look at my new assets, I immediately fell In love with them. I ate a delicious sandwich and had some coffee and was ready to head back to the hotel! Over All the pain was so so bearable, the only pain I really felt was when I tried to get out of bed and into bed, and also putting clothes on so I had to have some help with those things. Also waking up my chest was very tight and a bit sore from muscle inactivity, but as soon as I moved around a bit I felt super ! No need to take antibiotics at home which was awesome to hear. Off the pain killers by day 4 and able to get up on my own as well.  My advice to a smooth recovery is taking vitamin c and zinc, and also eating very clean - cutting out caffeine, alcohol and unhealthy elements at least 3 weeks before... I've heard many ladies on real self say they had a rough recovery and were in a lot of pain, so I'm not sure if doing the above things influence that, I know everyone is different.  Would 100% refer Dirk as an excellent surgeon who really specialises in the natural look women go for ! Will post an update once they "fluff and drop" a bit more for a final view on the end result Updated on 19 Jan 2021: Feeling stronger and stronger every day. Able to get up on my own, just this breast band is so annoying. Sleeping is getting much easier and so is cooking etc....the incisions dont look bad at all which I'm relieved by Updated on 20 Jan 2021: I've attached a before photo, so far I'm doing well and approaching day 9 of my recovery. Still some muscle inactivity in the mornings...But no need for pain killers at all which is nice. Missing getting back to exercising! Still taking vitamin c and zinc

Had such an amazing experience with Dr. Lazarus and his team. I was previously a 32A and he inserted 285cc in each breast. His team was caring and compassionate and just the best. They could not of done anything more to make me feel more comfortable in a nervous situation. Surgery was two days ago and I am in no pain at the moment, he has prescribed me medicine that doesn’t make me feel grumpy and doesn’t make me sleepy. If you are considering breast surgery, this is why I would DEFINITELY and WITHOUT A DOUBT, recommend Dr. Dirk Lazarus at The Tranquility Clinic in Cape Town: When I stop to really consider my personal journey and relationship with my breasts, I feel somewhat conflicted. There were times in my life when I absolutely LOVED them and then times when I couldn’t bare the sight of them!! The most memorable of the good times was when Mother Nature blessed me with the ability to feed, calm, bond, nurture and give sustenance to my beautiful baby boys...breastfeeding for me was the ultimate joy. After feeding my second baby, my breasts looked empty, saggy and sad, but nothing that a quick surgical procedure to insert implants couldn’t help. And it DID help, and once again, me AND my breasts were happy again and I loved them.The earlier times in my life when I hated them, were mainly in my teenage years, when I was teased because of how large they were (I’d developed so much earlier than girls of my same age and large breasts were genetically hereditary amongst the women of my family).But nothing could’ve prepared me for the way in which I felt about them in these more recent years of my life, especially the massive changes which occurred in the last two years. I’m a 47 year old woman who breastfed two babies, had breast implants almost 18 years ago...and then, much to my disdain and anguish, I hit my middle age spread and gained 15kg’s in two years!! What this weight gain did to my breasts was nothing short of SHOCKING. What the sight of my own breasts did to my body confidence and self esteem, was even worse!!! About a year or so ago, I began to ask around about recommended doctors that could offer me a solution, but for some or other reason, none of these recommendations gave me that “gut feel” of assurance or certainty...even after scouring dozens of surgeons websites and reading their reviews, I was unable to make a confident decision. Whilst visiting my mother in Johannesburg last year, she phoned a breast surgeon that she knew of and asked whom he would recommend in Cape Town. Without even so much as a seconds pause, he answered in one short, simple sentence: “Dr. Dirk Lazarus”. And MY “gut feel” kicked in and I immediately knew that Dr. Lazarus was my guy!!!I made my first appointment in early September of last year (2019), went for a mammogram and booked my surgery for the second week of October 2019. My procedure would include the removal of my implants, a breast reduction, a breast lift and resizing and shaping of my nipples and areolar...!!! Sounds scary huh??? Well, on the morning of my surgery, the VERY last thing on my mind was the fear of waking up with pain or discomfort... on the contrary, I had butterflies in my stomach I was so excited. I knew that I was in the skilled hands of an extraordinary surgeon and his equally extraordinary team of anaethetist and nurses...and I knew too that I would be waking up with beautiful breasts. That is HONESTLY the unquestionably large amount of confidence that Dr. Lazarus had instilled in me during his very thorough and extensively informative pre-op consultation...And that is EXACTLY how it all panned out. I still have decent sized boobs, but they are GORGEOUS boobs and they are ALL MINE...no more foreign implants. The pain and discomfort was an absolute walk in the park - remarkably minimal and VERY manageable. I’m almost four months post surgery, the scars are healing nicely, I have complete return of sensation, AND the surgery has been a great catalyst to me losing weight... so all in all, I AM FEELING LIKE AN ENTIRELY NEW WOMAN!!! It is at this juncture that it needs to be said, that the most unbelievable aspect of this journey to me being 100% content with my breasts, is to say that 5 months ago I walked into a doctors room for a consultation, and now, fast forward to today, I truly feel as though I didn’t just gain new boobs, I gained a new family too. He surely went above and beyond and exceeded my expectations by 100000 miles! He is brilliant in what he does, excellent clinic it was SO super clean, the staff were absolutely wonderful and overall I had a 7 star stay. I'm a week post op and things are good, just had dressings removed today and scars healing well. Dr Lazarus and his team were amazing, made me feel comfortable. He listened during my initial consultation and understood what I was going for. Having lost a lot of weight over the last year, I was not feeling like myself. Breast augmentation was not something I'd ever considered before, and even now I was happy enough - it wasn't causing self-esteem issues or anything. I just thought it would be a nice extra bonus. Didn't want anything dramatic. Healing has gone well, was off painkillers by day 3, almost no bruising, they're already softer and more natural looking than day 1. All in all, I think the key for me was understanding what I wanted, having a skilled and empathetic doctor who listened and who I could trust, and also having reasonable expectations myself. I didn't want to look entirely different, or have something that didn't fit my body. I did loads of research on what to expect and I feel I'm just allowing the process to happen and not stressing over daily little changes. I ended up needing 275cc in one side (our original plan) and 300cc in the other, to even things out. Happy with the results!

450cc - Fantastic Results, Quick Recovery

Its been 2 weeks after my surgery and I feel fantastic, I’m VERY happy with the results!! I had 450s put in, minimal bruising and Quick recovery... after 2 days I was up and about. I have always been aware of my deep desire to have more "normal" looking breasts but never really understood the nature of my condition.
