Should I take Accutane if I only have mild acne? Will my acne go away more quickly if I take it since my acne is mild?

 They are many reasons for acne including genetics, environment, hormones, makeup, stress, and medication side effects to name a few. Most important is to be screened for treatable causes. We commonly do not use Accutane for mild acne but for cystic and severe cases. Acne treatments should include a topical regimen of retinoids and antiinflammatories, in addition to oral medications if indicated. Chemical peels and easy laser treatments(i.e. Isolaz) can also be used to control your acne . Remember to protect your skin from the sun everyday to help the acne marks fade faster.

Could this scar/ depression on my nose be easily removed with surgery? (Photo)

This scar has two issues. The first being the hyperpigmentation ( darkness) over the scar. The pigmentation can be at multiple levels in the skin. Topical creams ie- vitamin C, glycolic acids , retinoids used in combination with daily sunblock will help to some extent  but  laser systems targeting brown - for example Clear and Brilliant, Fraxel, YAG will help lighten.The second issue is the depression at the site. Injecting a dermal filler, consisting of either hyaluronic acid or Calcium Hydroxylapatite ,under the site will lift up the depression. Fillers also stimulate collagen production for long term benefits. If the depression is not very deep, you may only need subcision. Either way, restart your daily sunscreen .

Best laser for hypertrophic scar/surgical scar? (Photo)

There are multiple ways to treat this type of surgical scar- cortisone injections , lasers and topical silicone products work well. This scar should respond well to a combination of pulse dye laser treatments and possible Fraxel combination ( most systems work very well). Multiple treatment sessions are needed. Your scar should do very well with whatever treatment you decide. Remember to protect the scar  from the sun while considering your options.

What makes Restylane Lyft different from Perlane?

Restylane Lyft and Perlane are the same product made by Galderma. It is an excellent product for lifting the cheeks but can also be used on the lower face.

Dog ear on neck scar. Is it normal for a dog ear on scar to look swollen at times? 11 weeks post op

It is very normal for a scar to  be intermittently swollen for months after the initial surgery, especially in an area that is constantly moving. ie. neck. If the scar continues to swell or causes you pain, please see your original surgery. You may need a scar revision ( removal of the dog ear) or a cortisone shot

Is it safe to have botox while using tretinoin cream?

Botox and Tretinoin are very safe when used together. A  retinoid , i.e  tretinoin , is an important part of your skin care regimen. Apply a small amount to clean skin at night and follow with a moisturizer over it. Initially, start every other night. If you have sensive skin begin every third night. It is important to use a sunscreen every morning as your skin is slightly more photosensitive. You should be using a sunscreen anyway to prevent additional photodamage. A good topical regimen will enhance the effects of Botox.

Does microneedling enhance lips?

Microneedling is a good option for stimulating collagen. The combination with topicals ie hylaronic acid, vitamin C, retinoids, peptides... gives the best results. Microneedling is excellent on the rhytids (wrinkles) above the lips but should not be used on the lips themselves. Remember to take an antiviral prior to the procedure if you are prone to cold sores.

Can Isolaz work for Adult hormonal acne?

Yes, Isolaz is excellent for treating all types of acne. The combination of intense pulse light and a vacume help to treat the bacteria and stimulate collagen to heals the marks. Isolaz needs to be used in conjunction with acne therapy--i.e. retinoids, glycolic acids, benzoyl peroxide, aldactone, possible birth control.

1 week post op fat grafting and I look the same after minus some asymmetry. How long before I can do filler?

It is important after fat grafting to wait 3 months before adding any additional augmentation to your face. A little asymmetry is normal after fat grafting. If at 3 months, you are still uneven, different fillers ie hyaluronic acids, radiesse  or sculptra maybe used. The choice of filler will depend on which area needs additional augmentation. My best advice is to wait for now as faces tend to settle nicely after fat grafting over a few months.

How to remove permanent knee and leg scars? (photo)

Scars on the legs can be difficult to treat especially on the knees, a constant bending area. Improving these scars involves1) a combination of laser treatments ie Fraxel and pulse-dye  and 2) a topical home regimen of retinoids and silicone scar gel. 

Multiple laser treatment sessions are needed as well as strict sun avoidance to the area. It's no possible to remove them completely but the scars will be less noticeable.

Fraxel I Liciously AMAZING @61 Years Old. - New York, NY

This was my 2nd Fraxel on my neck, chest and this Monday, I had the first treatment to the face. I had major pigmentation damage due to years of laying in the sun, which Ive now, sadly, had to give up. Personally, I had some minimal discomfort from the Fraxel re:store treatment. Prior to the laser, lidocaine was put on for one hr to the parts to be treated. While recovering from my first Frazel 3 months ago I had a rough time and experienced a lot of red blotches. This went away quickly and can be part of the healing process. Majorly reduced the age spots, tightened the skin and is also helping my minor lines. This time just bronzed, lizard like skin. I haven't started to peel off yet. Im thinking in a few more days. I initially looked like a had a sun burn and now just a scaly bronze to my skin. My neck experienced an amazing restoration from the first Fraxel and the lay turkey skin completely went away. It takes a while for the collagen to rebuild so the skin can retract. To really experience the amazing result you have to give the skin time to rebuild. I am convinced that this Fraxel is the miracle we have been looking for. Also this is procedure that really should be repeated to get flawless results. I will go back for the third Fraxel in a few months. I can't write about this most recent treatment because it just was done. Ill post monthly as I start to recover. Updated on 11 Jan 2014: All is going well. I have just begun to start to shed some of the lizard like bronzed skin. I have no pain but I am not being social this weekend. Figured it'd be best to lay low until Ive shed into my new skin. As far as a 3rd face, neck and chest treatment, time will tell once the skin starts to build collagen. Also need time to evaluate the age spots after I shed. Updated on 26 Jan 2014: It took a solid week for my facial skin and chest to start to flake off. It happens so slowly that only by touching your skin can you feel this shedding. My face still has some spots and now it needs the months it takes to restore and rebuild the collagen. I will need more treatments on my face. I spent YEARS in the sun so Im not surprised that this is the case. My friend has had many facial fraxel treatments, she's 63. I swear it looks flawless. Smooth, spotless, tight, bright, and very minimal lines. These treatments aren't a one time deal, until you're seeing the results you want. Stay OUT of the sun. BooHoo. If you go without 50 block those spots are right there, just waiting to pop up again.

Prep Before Filler Injections - New York, NY

Ive been using Juvederm and had excellent results. My problem was the bruising. I looked awful for many days after. This time I used Arnica dissolving tabs 6 times a day and substantially less bruised. Ive noticed just very light bruises at the entry site. I stopped after the injections. Next time Ill prep like I did this time with Arnica and continue for 3-5 days to see if that makes the bruising heal faster. I bet it does. Also never use aspirin or Advil for one week prior to treatment.

I had the new Thermage on my 59 year old neck and jawline just today. Dr Diane Madfes NYC who is an amazing cosmetic derm who worked with Dr Pat Wexler gave me something she KNOWS will work and it did already. I have noticed a tightening immediately with better results in time. Dr Madfes isn't one of the md's who push the latest fad lasers- i.e. Zerona etc but she had no doubts and now neither do I. Go for it!!!. Next time I'm doing my whole face. FYI it does burn slightly but only for a second with every new spot. Nothing intolerable. 5 stars.

Broken Capillaries and Real Health Concerns - New York

I've never been so disappointed or disturbed by a doctor of her office. Many reasons, but I'll focus on where she ripped me off. While at her office on 2/15/16 I asked her to zap a few broken capillaries. I was charged $300 (I was to have it done more effectively and for a fraction of the price elsewhere). I had a follow up on 3/18/16 and showed her that they were still there and not even faded. I was shocked to be charged another $300. I called her office immediately when I got home and saw that the laser hadn't even touched the broken capillaries. I also half expected to be told that the second charge was a mistake (seeing as I only needed it again since the first didn't work). They did not say that. I will spare you some of the details and tell that I've left 6 messages and she hasn't called me back to discuss the charges or my health concerns. I've been seeing her for 10 years and March 18th was the very first time I've called for any reason other than to make an appointment.
