Yes this can definitely be done at the same time, but it is usually done in 2 separate operations. First is the reconstruction with expanders or implants (in your case they should hopefully be able to do direct-to-implant and skip the expanders), and then after the implants settle into position, perform fat grafting artfully and skillfully to correct minor symmetries or provide better projection where needed.
