From the picture you show I believe it to be very unlikely that you would require a true mastopexy at the same time as your augmentation. A small amount of "lift" will occur with the implant alone. The implant will provide a nice improvement in upper pole fullness. The only reason for a lift that I might anticipate is to correct any slight asymmetries in nipple position that may be present after the placement of the implants, but this could be accomplished through excising a small crescent of skin above the areola and does not add much time or recovery to the procedure.
