Most implant reconstructions after mastectomy will place the implant under the muscle because it needs to have additional protection. In fact it would be highly unusual to place the implant just under the skin as it needs more coverage to limit issues with palpability, rippling, exposure, and unusual contours. Your incision placement will depend on the comfort level and expertise of your breast and plastic surgeons. Common incision sites are just above or below the nipple and in the inframammary fold. The inframammary fold incision is nice in that it is the most concealed of those options. Given your age, you may want to look into using your own tissues to recreate the breast if you have enough stomach skin and fat (e.g. a bilateral DIEP flap) just as an alternative option. Good luck!
Most of the associated swelling with lower eyelid surgery should have resolved by now. If there was fat grafting involved, this lump could be related to excess fat that was placed. Or if no additional fat was moved around, too little fat may have been taken out in that area. Without pictures it is hard to know, but I would still recommend returning to your surgeon to discuss the issue and its possible cause. Often times there are ways to correct such problems.
You may be experiencing an intense nerve-related pain that can occur after Coolsculpting. It is not very common but it is a known side effect, especially if you were treated with the larger CoolMax head. You should return to the facility that performed the Coolsculpting to have them evaluate you and determine if you have this problem. If you do, it can be treated with nerve pain modulators like Gabapentin (aka Neurontin).
It is important to focus on a couple areas that are important to you and then have a conversation with your plastic surgeon about what can realistically be addressed. For example, do you feel like you fill out a bra you like appropriately? If you wear dresses that expose a part of your cleavage, does that area look good to you? Identifying potential problem areas in this fashion allows you to ask what can be done. Remember to write down your questions so that you don't forget to ask them when you are there at your appointment.
It's a common misconception that liposuction will remove pounds (of weight) as it removes your fat. Fat actually weighs very little and it is more common to see a slight increase in weight due to fluid shifts and fluid retention after the surgery. At 4 weeks you should start seeing a return to your baseline but it can sometimes take 2-3 months to normalize.
