Wanting a long lasting solution is a great thing, but it comes with some challenges.  The first is matching the other front tooth exactly.  A highly trained cosmetic dentist could do that, but what happens when your natural tooth changes over time? Natural teeth do change  for a variety of reasons, including how light goes through them. You would still want the two front teeth too match exactly, but the crown would stay the same and the difference would be noticeable.  In my view your options are to use resin bonding materials to repair the chip on the one front tooth and replace it periodically, or do porcelain veneers on the two front teeth.  Veneers are much more conservative that a crown, you do not have to take away as much tooth structure, and if you do two teeth the front teeth would always match.  If it werre me in your situation, I would go with the bonding.  Properly done, it can last a long time.

You are not alone. We see this all the time. Fear keeps many people from taking care of themselves.  You can do the mouthwash thing, but it probably wont help, but it wont hurt either.  Your teeth and overall health will continue to deteriorate until you are forced to do something, only your choices will be worse and more expensive than convincing yourself you can do something now. 

Check websites for Drs that offer NuCalm, Sleep Dentistry, Oral sedation, or IV sedation.  If they have gone to the trouble and expense to learn these procedures, they probably understand and will be sympathetic to your position.  And can offer solutions.

There is a way for you to get the help you desperately need in a way that works for even the most fearful patient.

Totally agree that implants are your best option.  In the past we used to do a lot of heroic dentistry so people would not have to wear removable things in their mouths.  Now there are much better options

Without a photo of your whole face it is difficult to tell. For a really great smile the teeth size needs to be in the proper proportion to the rest of your face. I suspect that the previous advice is correct and your best option is an Orthodontic solution. Look at how narrow the bicuspid ( middle back teeth) is. Your smile would probably be prettier if that area was widened, creating the space for your front teeth to fit properly. Compare your smile to Julia Roberts. I think you could get there.
