What is the normal procedure to replace a crown that needs to be replaced due to an infection above it?

Is the root canal complete?  Sometimes it does take additional appointments to complete a root canal and you do not want to start the process for a crown if the root canal treatment is not complete.

If it is complete maybe the dentist is being cautious to make sure the tooth is not symptomatic.  Or, if insurance is involved, he may be waiting on predetermination paperwork from the insurance company.

Every case is different.  Those are just a few general reasons that may be holding up your case.

Water pic and cleaning between implants with pockets of 4 and 5?

Water pics are great but do not replace floss.  There is even a product called Hydro Floss, similar to a water pic...but still NOT a floss replacement.  Don't stop flossing.

If you are not already I would also suggest a mechanical tooth brush such as a Sonicare.  I love mine and recommend to all my patients.

There is also another treatment that can possible help called "Perio Protect".  They are custom made trays used with gel for 10-15 minutes a day that show real promise in helping the gums.  Ask your dentist if you are a good candidate for it.

Is it normal that a molar crown leaves a gap?

That outcome is not ideal but at times happens.  There are limitations to the implants and many other factors that can affect the outcome.  We, as dentists, try to minimize those factors but sometimes just can't.

Just going by what you stated and not seeing a picture, x-ray, or knowing pre-op condition I probably would have said the same thing as your dentist did.

Had 2 lower implants put in on May 8th. This morning on may 16th the left one popped out. Can it be reinserted?

Many times it actually is not the implant that fell out it is the healing cap or cover screw.  Before you fret about losing an implant call the dentist that placed it and have him assess it.  

If you have the opportunity take a picture of what actually came out.  Even forward that to your dentist and he may even be able to let you know before you go in.

Can a lateral porcelain crown be recontoured our debulked after cementation? (Photos)

Re-contouring is a possibility in many cases but there are limitations.  The thickness of the veneer is one limitation.  Another is the shade...and how the color is processed at the lab.  You may, by re-contouring, correct the shape but alter the shade in the area that was touched up.

It is a lot less traumatic than going through the entire process again and I would try re-contouring one veneer before re-preparing all 4 teeth...again.

A good number of people have the same problem that you are facing.  And, unfortunately, there is no "easy" fix and you need to find out what is involved with all the options before you choose what to do.

Your first step should be to get an orthodontic evaluation.  Just from the picture that you present there seems to be a lot of crowding on the upper AND on the lower.  Moving those teeth and lining them up properly may be all that you need to do.  But, don't expect an overnight process.
