The worse recommendation he performed was masseter reduction which for most males lowers their looks as one needs wider gonial angles and well-developed masseters for good facial width and dimension. Additionally the original genioplasty was crooked and the tissue looked mushy. So eventually I got a revision with the doctor again, and while he made the chin more symmetrical and more normal looking, I got the chin reduced which I only wanted like 1mm reduced and that was a big mkstake, as it got reduced in vertical and horizontal dimensions and made my face look fat and without angularity. I just wanted to get close to my pre-op which the doctor stated we can get close and we didn’t. At first it looked decent because of the swelling but then I realized it didn’t look as expected. I wish I never got the surgery, and it was the beginning of a surgical nightmare that really hurt my self-esteem and life path. I used to be called handsome regularly before, and the surgery made me ugly which I have to accept now. I just don’t think the doctor had good aesthetic vision for me and I came out worse looking. Updated on 22 Sep 2023: My face after all this was left asymmetric, different face shape that looks feminine as my masseters and gonial angles were resected creating a narrow face that lack angularity and width and of symmetry, additionally loose skin on mandible from lack of bony support. I used to feel confident, now I don’t and it all started with Dr Takahashi’s surgical procedures that changed my sense of self. If you are male, I would be careful with additional procedures he last minute recommends that are used for facial feminization, leaving the face looking feminine and lacking masculinity and harmony.

Evaluating you profile and frontal projection, I find that you have a weak chin projection and an asymmetric chin. I would suggest to perform a sliding genioplasty for midline correction and profile enhancement. Regarding your TMJ, we need to assess the diagnosis very accurately to determine if you have condylar hyperplasia. If that is the case you would need a specific treatment forecast. Please be adviced not to perform bucal fat pa reduction since it would ruin your aesthetics without performing genioplasty
Good day, Genioplasty definitely is a very powerful procedure to enhance the lower third and define the middle third of the face. Reviewing your photo I see that you have a class 2 profile. So after orthodontics without Orthognathic Surgery , sliding genioplasty is your only viable option. I would perform a two step genioplasty to gain a good advancement and a good profile enhancement. Genioplasty is a very powerful procedure to enhance the lower third of your face. The dent that you are seeing is the consequence of the gap created by the osteotomy. That gap can be filled either with temporary fillers while bone grows and fills the site, or a custom made iimplant can be placed to make the contour of the chin accord to the mandibular bone segment. The use of these devices for Orthognathic preparation is gaining popularity around the world . Nonetheless I firmly believe that you should have traditional ortho installed prior your surgery given that it’s easier for stability to be mantained trough the wire arch than splints. There are many surgeons that don’t do that and have had success. But cases should be very well selected. Talk to your surgeon about it beforehand. Best wishes
