The answer is of course you can. 

There will be a discussion of your wants.  I know this sounds strange but there are lots of options.  I always feel you should be involved in the decisions of what is being done.  This is you and your body, I feel you should be involved.  Depending on your wants will dictate what is necessary to get there.  Eg: do you want some thing that comes in and out or stays in?   There are so many options I am confident you will find out what is just right for you.  Ps My oldest patient to get implants was 93. You are not to  old and I am sure a great candidate for implants.

When doing an anterior case I always have a discussion of wants.  That way we are on the same page.  Sometimes it may just be a "copy" of shape, size and color.  Other times it is for cosmetic reasons for the changes.  All these changes are reviewed up front to make sure I can provide what is wanted. 

As far as the bite is concerned.....It is usually a simple fix of adjusting the crowns to match your bite.  When we are placing crowns sometimes a return visit is needed to verify the bite.  With everything new  sometimes it is hard to tell.

The location of the lab does not matter.  I am located in the North Atlanta area.  I use some labs in town, out of town and other states.  I have different labs do different things depending on their strengths.  The location is unimportant, the quality is what is important.

You are right. Elective dentistry can be very expensive. 
Depending on the amount of damage to your tooth structure you may just need recontouring (polishing the edges to make them look straight and as they should be), bonding,  veneers to crowns depending on the amount of damage done.  and each has associated costs. 
A shorter time treatment may be able to be done to get you smiling but knowing that in the future more definitive treatment will be needed.
Just like the other dentists we offer Carecredit for financing of treatment so hopefully you can get the treatment completed with financing.
As far as the other tooth is concerned I would recommend getting it looked at since it has a great potential to start to hurt.

There can be a few solutions to this problem. 
It may be something as simple as a minor bite adjust.  If your bite is "off" you may be banging the tooth every time your teeth come together.  After a few days you may have developed a bruise on that tooth.  If that is the issue adjusting the bite is the solution.  Remember it will take a few days for the discomfort to settle(waiting for the bruise to heal).
It may be the nerve may be dead(or on its way) and starting to abscess.  If that is the case a root canal is the solution.
On the off chance that the tooth is fractured on the root(remember it was already broken-the chips) then there is a good chance they may need a root canal our possibly an extraction.

I would definitely guess one of the first 2 options.

There are lots of potential reasons why you are having pain in your ear. 

One of them can be from your temporary crown or the tooth. 
Here are a few other examples: If the tooth is abscessed you can get pain in your ear depending on the location of the crown preparation. You can also get pain in your ear from holding open too big or long. 

More often we see someone with a sore jaw after treatment it is often to a bite issue.  The preparation and the temporary crown were fabricated when you are often numb.  On occasion the bite will be slightly different when not numb.  If this is the case and now you are banging onto it when biting it can put pressure on the jaw joint and can cause pain.  Often a simple bite check and adjustment will relieve the discomfort.
Note:  If you can do anti-inflammatories(advil/motrin) they will often help the joint area heal.
