I would have to agree with other physicians responding to this question that usually a combination treatment approach is best for under the eyes. If you are concerned about "hollowness" or loss of fat/volume under the eyes, filler would most likely be the best option - this is an easy and fairly pain - free procedure with little to no downtime and the results last around 12 months. For fine lines/wrinkles, we often use micro-needling and PRP to get the best results. With the Eclipse MicroPen, we are able to go extremely close to the lash line to target fine lines. Often, we are treating our patients with both filler and micro-needling devices to treat both issues and get the best results.
There are many different MicroNeedling technologies on the market. The main difference is the depth of the needles and this is where safety comes into play. I would recommend that if possible, you have your treatment done by a medical professional. Micro-Needling causes micro-injuries to the skin causing a healing response which produces growth factors and collagen. Since you indicated you cannot use PRP with microneedling, perhaps you can use pure Hyaluronic acid to plump the skin and diminish the appearance of pitted acne scarring. I always recommend my patients do a light microdermabrasion before microneedling is performed to eliminate surface debris.
While the CoolSculpting is still causing cryolipolysis (death to the fat cell by cold), administering the massage immediately helps to ensure that all possible treated cells die. By not administering the massage until after the area begins to return to regular temperature, you can risk less efficacy of the treatment. At our practice, we make sure the technicians are in the room and ready to remove applicator and perform the massage 1-2 minutes before the treatment ends.
Although the projected time frame to see results from CoolSculpting is 1-3 months, we find that our CoolMini patients tend to respond faster. This may not be the case for everyone, but in our experience, the submental fat is responding to the treatment in as little as 3 weeks with great results appearing even at 6-8 weeks. Wrinkles may show at rest or show with animation of the forehead.If they show with animation only, Botox is your best best. Don't treat forehead dynamic wrinkles without treating the frown lines between your eyebrows otherwise you will probably get ptosis ( drooping of the eyelids ) If the lines are too deep, you might benefit from having fillers injected into these lines as well
