The exact cause(s) of capsular contracture (CC) are still not completely understood. The use of Strattice and other like materials has proven to be a valuable tool in breast surgery, both cosmetic and reconstructive. BUT its use as a "sling" in patients with" bottoming out" is no guarantee that these conditions will not reoccur.CC is really an internal scar and its causes and subsequent bottoming out may be reduced by reducing implant size or type .What is your hgt. and wt. and original bra size? For now sit tight.
This is always the key question in rhinoplasty. You have great facial architecture and interrelationships, but when I blew up your photos there appeared to be a "butterfly" rash on your nose and cheeks that should be addressed before considering surgery. In the right patient who understands the pros and cons of combining proceedures it can be a good treatment option.
In my experience (40 years) this would require at least a 2 staged reconstruction: 1. You have a very severe 'unitized" scar of your mid abdomen. In lay talk that means that the skin, subcutaneous tissues, and fascias and muscles are fused in one unit so they cause a severe indentation at rest or when you tense your abdomen or cough. This needs to be reconstructed with a multilayered revision. 2.After this has healed liposuction of the abdomen and flanks can improve your shape. Some of the fat might even be used as a graft to "polish" or improve the scars if need be. As others have said it won't be perfect but in the hands of an experienced BOARD CERTIFIED plastic surgeon it can be improved.
You hit the nail on the head when you said " I want to be proportioned". Don't get hung up on the number of cc.s. Breast diameter is more critical in determining size and that will determine the volume of a properly PROPORTIONED BREAST--and slope (teardrop shape). Contoured prostheses can shift and be problematic. In my experience in over 30 yrs. I briefer smooth round prostheses, either silicone or saline as per patient's desires and remember smaller is usually better because what's an asset at 22 may be a debit at 42.
