I'm 23 years old and measure at a 34D to DD. I am 130 pounds. What implant size should I get?

You are right - you will be pushed well past a D if you go for the implant.  Without seeing you it is hard to make a decision, but if you are droopy, you need a lift.  The 455 cc implants will add approximately a pound to each breast, and I personally think your breasts will be bigger and droopier with an augmentation.  I also think the ultra high profile will give you a great deal of projection but will not fill out the superior pole (top) of the breast, and that is where most people lose a lot of volume when they start to droop.   It’s only my opinion but I think you will be unhappy with just an augmentation and you need a lift (with or without a small implant).  Yes, there are scars on the front of the breast with a lift but they tend to blend in well over time.  In doing the lift, the skin is tightened, so it may seem that you have a slightly smaller breast, although it will be tighter and higher (although perky is very hard to achieve!).  Sometimes the top of the breast is still hard to fill out even with a lift so you may want to add a modest implant - but I would go with a slightly flatter one to fill out the top without giving you overly full roundness at the top (‘boob job’ look).  Good luck!

Donut lift for the 3rd time?

All lifts can relax to some extent, but the donut lift has a little more tendency for the nipple to splay out (stretch out/enlarge) over time.  I didn’t totally understand when you said ‘2x my areola’ but i’m guessing that is what happened to you - the nipple stretched out.   Adding the vertical incision makes the closure more secure and gives far less of that stretching out phenomenon.  Scars on the front of the breast are, indeed, permanent, but those scars tend to blend in quite well over time.  I suspect that you will be back for a fourth time if you go with a donut lift this time around.  Good luck!

Can I get breast reduction to a point where it can give the appearance of a flat chest, or an AA cup?

It can be awkward and very uncomfortable to have asymmetric breasts, as you already know.  Breasts can be reduced quite small, but the limiting factor is when the blood supply to the nipple gets compromised.  One thought would be to make the larger breast a B-/A+ cup (rather than AA) and do a small bit of fat injection to the smaller side.  This would give the smallest bit of rounding out to the smaller breast but would also help create the appearance of a border of pectoral muscle which you are likely missing due to the Poland’s.   And you wouldn’t have to enhance the smaller breast if you didn’t want to.  With Poland’s syndrome you have a fair chance of having your surgery covered by insurance (no guarantees!).  Good luck!

Puffy Bags under eyes only when smiling. Any suggestions? (photos)

Hi there!    You have great eyes!  I think your changes bother you more than other people notice .. . that being said:  it looks like you have two things going on - at rest, you have a mild tear trough deformity (in the picture, it seems to show more on your right than your left, but that could be lighting .. .).  If that is what's happening, a judicious bit of filler would help fill the small hollow and lessen the shadow (my favorite is Restylane, . . . just don't let anyone do Juvederm near your eyes - causes very very prolonged swelling).  With animation (when you smile) you get a bit of bunching up of the lower lid - this is most likely from hypertrophic or thick muscle.  You could try a very careful bit of botox to see if you could decrease some of the activity of the muscle or you could do a blepharoplasty to actually remove (debulk) some of the muscle.  This would need to be done with an incision on the outside of the lid just under your lashes - this incision can hide very nicely.  A strip of muscle - just a few millimeters - can be removed to debulk the area and decrease the activity.  I would probably separate the skin from the muscle and might not remove any skin.  One has to be very cautious with lower lids to not cause increased scleral show (too much of the white of the eye showing) or outright ectropion (the lid pulls away from the globe) - those can happen inredibly quickly with overly aggressive tissue removal.  Now - a transconjunctival bleph keeps the incision inside BUT it only addresses the fat pad, and that's not your major issue and it would NOT fix the bunchiness.  As most surgeons will say, it is very hard to evaluate photos on the web, and you WILL need a personal evaluation before you can make final plans, but these are all my thoughts based on your pictures and questions.  Good luck!

Help with exchanging implants for a larger size!!!! Currently have 230 cc HP, thinking about 350 cc. Any suggestions?

As others here said, it is too early for a revision.  You should wait six months to let swelling settle and to let your tissues drape out a bit - if your swelling goes down enough, you might even want to go beyond 350 c.c.s!  A general 'rule of thumb' is that 100 ccs is approximately one bra cup size, so you can figure it from there AFTER your swelling settles.  And as for revising it under local, it might be possible to do it if your implants are over the muscle, but under the muscle is often too uncomfortable to do under local anesthesia.  Figure this - you wil most likely need some muscle work to open the pocket a bit more to accommodate a bigger prosthesis, and, again, that's more uncomfortable.  As for style:  If you increase size enough, the larger HP would be wider and give you a bit more cleavage, but if you really want to be sure, you might want to change profiles to a moderate plus style.  This has a little less projection, but gives you more width, which would add more cleavage and more volume up top.  When it comes time for revision, you could sit down with your surgeon and look at the various widths/heights.  Don't overthink it, though - it's just a few millimeters in any given direction.  My personal preference is Mentor, but each company has similar shape and size variations.   Good luck!

Saline or silicone breast implants? (Photo)

Saline and silicone look exactly the same from the outside in someone who is starting with as much breast tissue as you have, so don't worry about looking unnatural with saline.  Saline implants are less expensive and need a smaller incision, but they have a higher rupture rate than silicone and there is a texture issue, meaning they can feel like a baggie with water in it.  That being said - you are starting with enough breast tissue that it is unlikely for you to be able to feel THAT much difference in the implants, although you might notice it at the bottom near the incision where the tissue is thinnest.  I personally prefer the texture of the silicone - it's just more pliable (squishier!) and is just a bit more pleasant to the touch.  And while the old silicone implants ruptured frequently with very notable consequences, the new implants are manufactured quite differently and the known defects have been removed.  I have seen only one rupture in the last 15 years - it was completely asymptomatic, was found on routine mammagram, and was not 'messy' when we switched it out at surgery.  (Be aware that, with silicone, the manufacturers request that you get routine MRIs to monitor the integrity of the implant.)  My personal preference is silicone, but I do whatever the patient wants. So  to summarize - with the amount of breast tissue you have to cover an implant, you are highly likely to get a very natural appearance and a good texture with either saline or silicone (maybe a tiny bit nicer with silicone) - you don't need to worry about an unnatural appearance with saline or a 'mess' with a silcone implant -- and whichever you choose, you are going to enjoy it, because you have a lovely appearance already, and either implant will enhance that nicely.  Good luck!

Correcting your concerns is going to require surgery - there is nothing you can do on your own to tone or shape your breasts - I'm sorry.  

Both breasts are low, so you will require a mastopexy (lift) to bring the gland and the nipples higher.  Sometimes this leaves you a bit flat at the top, so you might want to consider adding an implant to round out the top.  If you decide to have surgery, you will have to choose which side has the best volume - do you like the size of your larger breast?  Then put an implant in the smaller one to make it as big.  Do you like the size of the smaller breast?  Then make the larger one smaller with a breast reduction!

So - there are several options to think about.  Find a board certified plastic surgeon in your area and discuss your desires.  He/she will examine you and discuss potential results, risks, recovery, and, of course, costs - then you can make your final decision.  Good luck!

p.s.  it's hard to tell from the foto, but is there a slight indention at the left nipple?  Are you up to date on mammograms?   If that's not the way you've always been, you need to get it checked out.

How to correct prior breast lift and augmentation? (photos)

The combination of breast lift/augmentation is one of the hardest surgeries we do, and it not uncommon to require some sort of revision several months down the line.  It would be good to see before fotos, a picture of you now with your full torso (from above shoulder to waist), and a picture showing the folding over when you lean to the side.  From what I can see, though, my thoughts are this:  
1.)  the implant seems a bit high and a bit too defined at the top.  This may or may not be true in real life - I think implants photograph more 'fake' than they actually look in person.
2.)   your cleavage is narrow - is it a lot narrower than it was before?  I think I see a bit of double-bubble along the inner aspect of your right breast - suggests the implant has been placed more toward the midline than your actual breast tissue.  Is this the side where the breast folds over the implant?
3.)  skin redundancy and slightly lower gland at the bottom of the breast
4.)  nipples are level (yay!  hard to achieve) and at a reasonable location - technically want to be about half way to 2/3 down the height of your upper arm
So - cannot tell if all of this is true without an exam.  Options - repeat the pexy and lift the gland up higher over the implant - definitely needs the anchor pattern if you don't have it already OR revise the pocket and bring the implant lower to fill out the loose breast and possibly a little more lateral to stop the folding-over effect.  You might still need some skin tightening with this option.  Bringing the implant lower into the gland MIGHT rotate the nipples up a bit without re-doing the lift, but you ought to decide if your happy with your nipple location as it is before you do any revisionary surgery.

But - these are just thoughts - it is impossible to tell what is happening from a foto.  Discuss it with your surgeon until you are satisfied, but keep in mind there are things that simply cannot be done surgically and that you might have to live with.  If you are unhappy, you can always go to another board certified plastic surgeon - but please give your original surgeon a chance to make you happy.   We take pride in our work - your happiness is part of our reward!  Good luck!

Do you think I could qualify for a breast reduction on Aetna insurance?

Start by seeing if your insurance company even covers breast reduction and whether it wants you to get a referral from your PCP to a plastic surgeon or whether you can just make an appointment.   Once you get to a plastic surgeon, he/she will review all your symptoms and do a thorough exam and may at that point submit you for insurance approval.  It is not uncommon for the insurance company to deny the request the first time around and ask you to do things to document your problems.  You might be asked to lose weight, go to physical therapy, etc .  Do what the insurance company wants, then follow up with the required doctor.  Know that there is a height/weight chart that suggests how much we have to take off your breasts to give you symptomatic relief, and many insurance companies follow that chart.  This means that we might have to take a LOT off your breasts IF your weight stays up.  From the sound of your symptoms, I think you would really like a breast reduction, so do NOT get discouraged - they really make you do a lot of things before you qualify.  Know that many women in the South weigh in at 250 and above at your same height and still get approved.  Good luck!

Do I have to see a doctor (other than a PS) for an insurance company to approve me for surgery?

Each insurance plan has different rules, and those rules can change from year to year, so you should start by checking with your particular plan to see if you need a referral from your PCP.  If you do, then yes, you'll have to see your PCP for an exam and a discussion regarding breast reduction.  If not, you can directly make an appointment with a board certified plastic surgeon.  Do not ignore the insurance rules - it might lead to future denials for this problem!   But ultimately you will get to a plastic surgeon who will do a thorough exam, list your symptoms, and submit a request for breast reduction to your insurance company.  It is not unlikely that the insurance company will deny the request the first time around.  Do NOT get discouraged at this point - simply do what they request and your chances of approval improve immensely!  Often they want to see that you have exhausted all conservative treatments (weight loss, physical therapy, pain medication/muscle relaxers, documentation of rashes with treatment used, etc) before you resort to surgery.  And there again, you might have to visit your PCP for some of those treatments or referrals.  You sound like you have the classic symptom complex  that would respond beautifully to a breast reduction, so I think this is worth pursuing, even if it is a paperwork hassle.  Again, start by checking to see what your insurance company requires, and go from there.  Good luck!
