Q: Is just a tip rhinoplasty an option for me?
The tip can be narrowed both by open and closed techniques. The open technique however, gives more freedom to narrow and reshape the tip properly.
From your pictures, it seems you have a boxy and wide nasal tip. The rest of the dorsal aesthetic lines are OK. If you do not mind the mild hump of the nose, just narrowing of the tip of the nose would be sufficient.
Q: I'm on blood thinners (rivaoxobane), can I have a BBL and tummy tuck done?
Without your photographs to accompany your question, it would just be a theoretical answer. It should be possible to do a bilateral breast lift and tummy tuck at the same time, even if you are on blood thinner. As you have done it before, we would have to stop your blood thinners a week to ten days prior to surgery. Ofcourse, this would be done only after your physician who has put you on blood thinners gives permission to do so. If there is any contraindication to stopping blood thinner, then I am afraid, the surgery won't be possible.
The other prerequisite is you should be fit and well, with a good Haemoglobin and MCH level.

Your front view photograph is not a proper frontal passport type photo. That is required to comment on width of nose and nostrils. from the profile, it looks like the the hump could be lowered marginally, to improve the profil tremendously. Your tip is also bulbous and could do with narrowing. Both these goals cn redone at the same time.
