I researched for the best rhinoplasty experts in Pakistan and found Dr. Arfat. I had a nasal hump and slight deviation in my bridge. The surgery exceeded my expectations. My nose is still healing and it is going to look even better in a few months. He is very receptive and pays close attention to your expectations. The surgery and post op was really smooth. Only 3 weeks have gone by since my surgery and I have become accustomed to the changes my nose brought to my face. The change was a bit of a shock for me initially but as the swelling went down, my nose looked better every passing day. He really is one of the best rhinoplasty experts in Pakistan. Only thing I think can be improved is post op check ins and may be an in office visit few weeks after surgery. Since the nose changes and surgery on face has a lot of psychological factors attached to it, i think it would've been even nicer if I got more consultation and more check ins. I travelled to Dr. Arfat for the rhinoplasty from the other side of the world. The communication with Dr. Arfat from the beginning was pleasant, we had a video call in the beginning and that was enough to in-still confidence. The main reason for choosing Dr. Arfat is that he specializes in rhinoplasties, and he cares about his reputation, for e.g. his online presence. Too many surgeons out there don’t end up specializing and so are never able to achieve the kind of results that a specialist can achieve. And if we are to compare him with other surgeons in Pakistan who have zero online presence, then he is your only option there. He performed revision rhinoplasty, where he fixed breathing issues and a hump. Cosmetically my requirement was that I want more definition (finesse?) in my nose, especially the nose tip. I’m about half a year post-surgery and I do still have swelling, which he has confidently mentioned will go down further and I’ll be able to see more definition in the tip and the supratip break I asked for. Dr. Arfat takes a conservative approach with the nose, where he tries to preserve the customers features and ethnicity, which is something I see most good surgeons doing in the US & Canada. For example, he ensured he does not rotate up my tip too much which could potentially change my face and especially my smile. I agreed with him. The nose looks great. After my primary rhinoplasty with another surgeon my tip had ‘fallen’, a hump appeared and my breathing issues returned after a year or two. I have confidence that this will not happen this time. Worth mentioning: Because previously my tip dropped, I was very anxious before the surgery and I told him that, but by the time we had to go to the operation theatre I had significantly calmed down. He also had another surgeon operating with him, which I don’t know if it made a difference but it was even more re-assuring for me personally. Now the results: I have a straight bridge, it looks very natural, the nose is slightly smaller than before (which I wanted) and I have a very appropriate amount of projection. I’m waiting to see more definition in the tip but I’m sure that will come within a few months. Summary: it is worth it. I spent 2 years looking for a surgeon online all over the world (I’m very conscious about my nose & it’s a source of anxiety for me as my primary rhinoplasty was not successful so I was willing to wait until I find the right surgeon for me) and he was my first choice. The results do not look like I’ve had work done, very natural results.

Surah dua I am in love with my self again! I had lost all hope! If you want a human with a heart first not just a doctor with tools then you have found him! He is absolutely an angel on earth, I swear! I had a botched nose job done from a self proclaimed best surgeon in New York City on Madison ave, he has 5 star rating on real self, which was all fake! My surgery was expansive and I thought If I was paying so much he had to know what he was doing. The day my cast came off I knew something was wrong, my nose looked crooked, he made it supper short and upturned, My columella was hanging, I had a notch in one of my alar rims, he left me with a open roof deformity, I cried all the way from his clinic home. I came home and started searching for doctors to fix the mess he had created, I remember coming across Dr. Arafat’s Instagram, I got in touch and he immediately responded with a video call. I fell into deep depression for many months after, I lost many of my precious 20’s in this. I work in the fashion industry in New York City and having a ruined nose job brought my self esteem really low. I want to take a moment n thank Dr. Arfat for the amazing results I got. My revision rhinoplasty story !!!! Spreader grafts is a versatile technique that no only straightens up the cartilagenous part of nose (lower two thirds of nasal dorsum) but also gives Burrows lines and prevents inverted V deformity. It's ideal to retrieve the cartilage from inside the nose however if that's not possible due to previous surgeries the it can be harvested from ear cartilage.
Yes we can make your nostrils smaller and decrease the alar flare. It should be the last procedure in a rhiniplasty as tip-plasty or increasing the tip projection sometimes decreases alar flare to some extent. Procedures for decreasing nostril size include alar wedge resection, sill excision and V-Y plasty in an incremental order. In alar wedge resection a precise wedge of skin and subcutaneous tissue is removed and the incision in hidden in the groove on sides of ala. In sill excision a measured amount of tissue is excised between the columella and attachment of ala on each side. It is usually combined with alar wedge resection. In V-Y plasty a small incision is given in alar groove. A hook is inserted to pull it out creating a V. It is closed in a Y shape to push the nostrils inwards from outside. You can discuss these options with your surgeon. All the best.
There are various types of external nasal splints including malleable metallic splints, stick-on splints, silicone splints and some surgeons still use ortho-plasters. Most of the surgeons remove them on 7th post op day. Removal of an external splint is never painful. I use malleable metallic external splints and sometimes remove them even on 3rd post operative day and re-apply after assessing the skin underneath. Your surgeon should have been more gentle in removing the splints. However if the nose looks alright then we should hope that the painful removal has not effected the final result.
It's sad that you didn't get the desired results from your previous surgery. You need to look for a doctor who is expert in revision rhinoplasty. I would go for an open approach with spreader grafts to straighten up the cartilaginous dorsum and osteotomies to straighten up the bony parts.
It's an old post but I felt a need to reply this as there are not much Pakistani doctors on this web site and many patients from this region may have similar queries. It's sad not to have a good outcome from a cosmetic surgery. You need a revision surgery addressing the callous on your nose bridge, open roof deformity and the bulbous tip. In Pakistan both otolaryngologists (ENT specialists - as we call them here) and plastic surgeons are licensed to do a rhinoplasty but not all are very keen on doing this procedure as the market is still developing and is currently not very rewarding for the surgeons. You need to choose a doctor who has a passion for rhinoplasties and has good reviews and results.
