There are two ways that moles can be removed for cosmetic concerns. They can be shaved flat or they can be cut out with an instrument or scalpel. Shaving works best if the mole is elevated because it can be made even with the surrounding skin and the scar is often subtle. If the mole is relatively flat shaving does not work well because we then have to scoop into the skin and you could be left with a divot. Cutting it out works best if it is flat, however, you will then have a linear scar. It would be best if you can be seen and evaluated in person. Based on the photo you sent and the size of your moles you should be getting skin exams with a board certified dermatologist regardless! I hope to see you soon.

What you have is phytophotodermatitis a condition that happens when you come into contact with certain fruits or vegetables, such as limes, lemons, parsley, celery, and are in sunlight. A reaction occurs on the skin that results in these dark patches. Were you drinking margaritas or Corona with lime? The good news is phytophotodermatitis is not dangerous and the dark areas will resolve with time. If you would like to try to speed the resolution of the patches you could try hydroquinone. Thank you for the question. It is difficult to fully ascertain from the photo, but I believe it is your right eyebrow that is slightly higher; however, it is best to have a formal evaluation. If one eyebrow is higher than the other, a small amount of Botox can be placed above the elevated eyebrow to help make them more symmetric. Best of luck.
It is safe to apply Retin A every day, preferably in the evening, indefinitely; however, hydroquinone should be used once to twice a day (morning and/or evening) for 3-4 months at a time, stop for 1 month, and then you can repeat if necessary. Hydroquinone has a rare paradoxical side effect called ochronosis where one can experience permanent darkening of the skin; this is avoided by taking a break from hydroquinone every few months. If you use the hydroquinone twice a day you can mix a small amount of it with the RetinA in your palm and apply them simultaneously in the evening.
In general, we recommend waiting a minimum of six months to do any lasers, peels or dermabrasion after finishing Accutane (isotretinoin). This is because Accutane decreases the amount of oil on the skin so there could be a higher risk for complications. With regard to the treatment you already had, it is okay to start the Accutane at anytime.
