Addressing my breathing concerns while also cosmetically enhancing my nose has been something I wanted to do for over a decade. For many years, I had issues with breathing through my nose, as well as, multiple sinus infections a year. I've always had a specific cosmetic goal I wanted to achieve when changing my nose shape. I am very particular when it comes to my appearance and I aimed to find a doctor/ surgeon that I could trust. I first heard about Dr. Patel from my brother-in-law who worked within UK. I had cancer in my parotid gland for 2 years because the Doctors thought it was Bells palsy. When tumors started to show on my neck they realized their mistake. I had surgery to remove the tumor in August of 2023. It destroyed my facial nerve and I came home with not only my sagging cheek and drooping eye, but one ear smaller than the other and sticking out from my head at a 45 dree angle. The surgeon had to take out a lot of cartilage along with a muscle in my neck and one from my shoulder. I didn't want to leave the house for 10 months. I did go to the grocery store and shopping with my wife, but I was always aware of how I looked to people. The first reconstruction procedure Dr. Patel performed was on March 8th, 2024. I got his name from the University of Kentucky Health Care site. There were only two surgeons listed for facial reconstruction but his bio spoke to me. Among other things, he had studied sculpture and brought his eye for human aesthetics into his surgery practice. For weeks after I made the appointment for a consult, I heard from several sources that he did excellent work. I read multiple reviews, all of them good. The one that summarized them all was a person whose review ‘voiced’ regret the satisfaction scale only went from zero to five. Five was not nearly high enough. It has been 10 days since my last procedure and I cannot agree more. It wasn’t until my second surgery, April 24th, 2024 that I felt like I got a big part of my life back. He raised my eyebrow, put a gold weight in my eye lid so I can now close my eye, tightened up my lower lid so that tears could be closer to my cornea and lessen the possibility of it drying out and losing my sight, shaped and moved my ear back, tightened my cheek up so that there is no visible sagging and my top lip no longer curls around my top teeth which distorted my speech so much I had to physically lift my face to be understood. The second surgery took the curl out of my lower lip so that I could stop biting it when I ate and made my speech even more understandable. I have informed the church that I am ready for jobs they may have for me and next week I plan to walk into the Hospice office and announce that I am back. Thank you more than I can express, Dr. Patel. But my story does not end there. For the things that surgery gave me, there are numerous things that cancer took away that no amount of surgery of any kind will give back. The right side of my face will always be seen in high contrast to the left, (wrinkles on one side, no wrinkles on the other). Half of my mouth will never move when I talk. My right eye will never open as wide as my left, will almost always be watery making it more conspicuous. With no muscle tone in my cheek I will always have trouble eating, will never whistle again will always have some degree of speech impediment. There is more, but you get the message. Life will never be the same but that does not mean that it can’t be as good or better than it was. If you are reading this you are most likely looking for answers to any number of questions. I had surgery in June to correct breathing issues and my crooked nose, due to an old injury. From the very beginning Dr. Patel and his Team made me feel comfortable and informed about the surgery. I can’t say enough good things about Dr. Patel and his staff. I have been extremely insecure about my nose for most of my life, and have always slightly struggled with breathing issues. I’m only 19 years old, so I was really nervous when i decided that I wanted surgery. On May 13th I had a Rhinoplasty with Dr. Patel. After turning 50, I chose to make changes to my face to help ease signs of aging for the upcoming decades. After several discussions, I decided on a brow lift, blepharoplasty, face lift, and neck lift. Dr Patel and his staff were courteous and responsive to my requests. Dr. Patel is a perfectionist in his work but very affable when communicating. He and his staff are prompt and responsive. Happy with the results. Significant improvements in my face but not overdone!
For 22 years I had breathing problems, I had a deviated septum & some other things wrong. On March 25, 2021 Dr.Patel changed my life forever. The surgery has been an absolute success and I love it so much. I can finally breathe all the way out of the left side of my nose. The healing process wasn’t very painful, just make sure you do your nasal rinses!! I can say the worst thing about it was I felt very congested from the packing and nasal cones but it got better day after day & I can say 20 days after having the surgery as I type this I can’t even begin to explain how amazing it feels, it’s indescribable! About 2 years before meeting Dr. Patel, I had my first unsuccessful surgery to correct a deviated septum. This was something I'd been born with that had worsened over the years to the point that I suffered from minor breathing difficulty and nearly constant sinus infections, despite treatment with the strongest antibiotics or any noninvasive procedures. After my first septoplasty, I was excited to finally know what it felt like to breathe normally. The result was the polar opposite. Not only did my breathing trouble worsen, but the infections worsened. I shrugged it off as "bad luck" and as soon as I was offered a 2nd surgery to revise it, I jumped at the opportunity, despite the fact that the recovery had been one of the most painful things I'd ever dealt with. The results of the 2nd surgery can only be described with one word...WORSE. Everything got worse. During early recovery, you're expected to sleep with your head raised to prevent drainage, etc. From the time of the recovery from the first surgery, until meeting Dr. Patel 2 years later, I never slept laying down in my bed and was restricted to a recliner because as my breathing worsened, I became unable to breathe laying down. I became depressed, unable to do even the simplest things due to shortness of breath. I can't tell you how much I took sleeping in bed for granted! It sounds like something so meaningless, but once you can't do it, it becomes so important. A month after my surgery with Dr. Patel, I was able to sleep without even snoring, much less any serious difficulty breathing. I can't begin to explain how wonderful that felt. At our first meeting, I was skeptical. I was convinced that there was no chance of improvement and that the PAINFUL recovery I'd experienced wouldn't be worth going through a 3rd time. Dr. Patel seemed confident that he could help me and I decided to roll the dice against a lifetime of breathing difficulty and the constant risk of infection, pneumonia, etc. I am so glad that I did. You never know how much you appreciate the simple act of breathing until you can't do it. And when you can't breathe, nothing else matters. If you are having a similar experience, if you are skeptical, if meeting Dr. Patel wasn't or isn't enough to convince you, please take my experience into account and give yourself the chance to breathe again. My last recovery was actually easier than the first two somehow...and now I'm able to breathe again, to sleep normally again, to not be constantly sick. It was worth it. Dr. Patel changed my life and gave me a chance of living a normal one again, nothing short of that.
In 2020 a visit to my dermatologist resulted in the discovery of a melanoma on the end of my nose. The surgery to remove it resulted in a hole on the end of my nose about the size of a quarter which I brought to Dr Patel to reconstruct. This is a scary series of events; melanoma is probably as bad a skin cancer as you can have and getting a big chunk of your nose removed is not much better. But this is the part of my story where things started looking brighter. Dr Patel explained my options and I chose the one that was not the easiest but had the best probability of a normal looking face when all was said and done. The first procedure he did was to remove a flap of skin from my forehead and while it was still connected to me with a small artery in it, he rotated it down and filled in the hole in my nose. This allowed the skin to heal while having enough of a blood supply. Now the hard part, 3 weeks of having this pedicle extend from about eye level to the end of my nose. It made me very self-conscious and I tried to not go out in public unless I had to. I told my friends that I was trying out for a star trek role as an alien. I wear reading glasses and it made it difficult to read or use my computer until the pedicle came off.Finally, I returned to Dr Patel’s office to have another surgery to remove the pedicle. Yeah, normal again and incredibly pleased with the results. Looking at me know you don’t even see it unless you have had it done to you, or someone close to you. So, I just went in for my one-year checkup. Looking back and asking myself would I do it again? Absolutely, three uncomfortable weeks to get rid of a potentially fatal cancer was well worth it.
I am so grateful that a local ENT doctor referred me to Dr. Patel for my septorhinoplasty. After only a few minutes of observation and hearing my story, Dr. Patel was able to tell me exactly why I have the breathing issues I have and what improvements he could make with my breathing and chronic sinus and allergy problems. Follow the recommendations of your dermatologist. Accutane can significantly affect the natural healing cycle at the cellular level. You should delay surgery for at least 6 months prior to rhinoplasty. Patience is a virtue in this case!!
Your question is a very valid one. Ultimately, the answer will be influenced by several factors but 4 important ones to keep in mind are as follows: 1. The amount of cartilage that remains from your first surgery and the strength of that cartilage 2. The degree of over-rotation you currently have and the degree to which you want it corrected 3. The technique that was performed to rotate your nose in the previous surgery 4. The degree of skin mobility you currently have following surgery. For ease of understanding, if you consider the analogy of your nose resembling a table cloth drapped over a table, the shape of the table cloth reflects the shape of the underlying table. In the same fashion, the overling skin and soft tissues of the nose reflect the underlying architecture of cartilage and bone. Following your first surgery, the skin/soft tissue "drape" conformed to the underlying cartilage/bone framework. Bringing your tip "down" will require movement of both the "drape" and the "framework" and so both will need to be considered. In many revision cases, rib cartilage is required to push the tip down in order to offset the scar contracture produced from a prior surgery. Revision rhinoplasty is one you should take seriously. Make sure you feel comfortable with your surgeon and with his/her experience in treating "over-rotated" noses & his/her experience with using rib cartilage for rhinoplasty. Best of luck!!
As a general rule, it is best to wait at least 12 months for a revision tip rhinoplasty. The tip of the nose has a tendency to retain swelling longer than other areas of the nose following surgery. Furthermore, several factors (ie the surgical approach, amount of cartilage grafting performed, your skin quality, etc) can influence how long it will take to achieve a stable result. You can take an active step in reducing your swelling by managing your diet (ie reduce your salt intake, reduce your exposure to personal food sensitivities). Best of luck!!
