Anyone maneuver on the nose will result and changes to other angles, distances or proportions for other aspects of the nose. By de rotating a nose, it will often look longer and more projected. The nasal width, however, is typically not affected by de-rotation of the nose. a good rhinoplasty surgeon takes all of this into account as well as the patient's own proportions and aesthetic requirements. It is also very important to discuss realistic expectations and outcomes of patients so that they are aware of and what to expect after surgery.

A known complication of rhinoplasty surgery is called a pinched tip deformity. This often results when there is overresection of the lower lateral cartilages. It is difficult to say from your photographs however, my advice would be to seek a second opinion from a board certified or board, eligible plastic surgeon, who has extensive experience in revision rhinoplasty.

Often times patients wanting a shorter nose from the profile really mean they want to tip deprojection. It is difficult to do this in the closed approach and results can be quite unsatisfying to patients and surgeons alike. The best approach for tip de projection is via the open rhinoplasty approach. A common complaint of patients wanting to undergo rhinoplasty is a droopy tip when smiling. Although this can be due to a multitude of reasons, occasionally it is over-elevation of the ala (sides) with a depression of the tip due to the depressor septi nasi muscle.
