Pretty straight forward and a common problem, you dont have to do all these, but some are easier than others.

At home skin-care regimen - Neocutis Lumiere Riche (or something like this) +/- very low dose retinol/adapalene.
In office laser treatments - aggressive or non-aggressive regimens, may take multiple sessions but worth it
In office dermal filler - must be done by someone who is skilled in this area, soft HA gel filler (this option will get you the furthest the fastest IMO)
In office neuromodulator - Botox to help decrease movement and make lines worse around the area.

I'm 45 and have what I believe to be malar /festoon mounds, Treatment options? (photos)

I would recommend a consultation with a laser or energy based device specialist (high end medical spa / Derm / Plastics) and/or a plastic surgeon for a recommendation on surgical options.

I think that the former (energy based devices) can provide some real benefit to help tighten the skin in that area, while surgical options may provide the greatest result.

I would avoid injectable fillers anywhere near this area. Fillers will only make it worse IMO.
If you have done injectable fillers, I would consider trying a dissolving agent in that area in an attempt to see if it improves, but based on photos energy based treatment is probably best (RF micro needling, fraxel, etc.)

Superficial Injection- how can I dissolve this? (Photos)

You must first have a proper history in order to determine the likely hood the bumps are filler or a reaction to filler. If they are HA filler (as you state), injection with Hylenex should usually resolve these issues. If the dissolver wasn't put directly into these "bumps" it wont work as well.
Furthermore, if these bumps occurred 2-3 months after injection and appear to be non swollen I would classify them as non-inflammatory DON (delayed onset nodule) which is more common in patients with autoimmune disorders. Many factors can contribute to this. In my experience; oral antibiotics, oral steroids, hylenex, and injectable steroids typically have worked for my patients in the past. Another method would be using 5-FU. I would try these before any other more invasive modality and consider biopsy.

please use a softer filler that is less likely to have this issue in the future and go slow.

Could this be residual Juvederm Voluma from a couple of years ago? If so, what can be done to fix my eye? (Photo)

Voluma is one of the longest lasting HA fillers in America and the world. I am not sure from your photo if it is filler. A proper in person physical exam can help make a better decision on the issue. Using dissolving compounds can help improve residual filler but you may never be able to 100% dissolve filler. I would imagine (based on the limited photo you sent) that there are other factors relating to the "puffiness", it may be worth a try dissolving to see if it improves.

I had botox for a gummy smile and now have a terrible joker smile. Can this be fixed by targeting other muscles? (Photo)

First and foremost, thank you for your photos. Without seeing a before photo it does make things a bit more difficult to assess, but I would say a few things: 1. things should soften up if a small dose of Botox was used rather fast (2-6 weeks). 2. Luckily your smile (although not your "normal" smile) looks pretty good! 3. You could always target another area, but the lower face is very sensitive to botox injections. I personally would let it return to normal as adding more has a risk of making things worse.

Hard lump under lip from being split open, what can I do? (Photo)

While this is rather common, most patience do not come in for this complaint. In the general scheme of things, there is still changes that will go on in the tissue over time. There may be some modalities like vigorous massage or steroidal injections that may break up any dense scar tissue. My recommendation would be the former and to let time heal your issue. You can always see a physician to better understand your concern with a physical exam.

Can I get lip fillers if I play the trumpet?

Yes you can, I would recommend considering a very natural filler vs fillers with higher risk of migration due to over-use while playing an instrument. I would assume that you may be at a higher risk for results "going away faster" due to over-use. Either way, you should be fine to get your lips done and play the trumpet.

How can I make my nose look less bulbous and prominent? Non surgical way (Photo)

While subtle filler may help decrease shadowing and indentations that are drawing attention to your nasal tip and nose, I do think that a surgical approach would be best for your scenario. Please consult with a few board certified plastic surgeons before proceeding.

Can I get a non surgical nose job? Or do I need to get a revision rhinoplasty? (Photo)

Thank you for your photos, I perform hundreds of non-surgical rhinos a year. I tell all my patients the same thing: 1) the gold standard for most people is a surgical correction 2) surgical and non-surgical results will never be 100% perfect 3) the frontal and 45 view (not shown here) is typically the hardest to correct/meet expectations. I think in your scenario you should at a minimum have an in-person consultation with a plastic surgeon before moving forward, so that you can better understand your options.

  2. Cheek Filler
  3. Laser Resurfacing
  4. Home Care with cosmeceuticals
  5. Other modalities (RF, ultrasound, and more)

The Best Doctor!

Dr Blinski is one of the most caring doctors, I've gone to him for botox, as well as aqua gold and I LOVE the results everytime. I've sent a few friends to him and they only go to him now instead of other doctors. He does my botox exactly the way I want it with eyebrows arched up a bit but always a very natural look and the aqua gold completely cleared my skin and made it look like glass, beautiful! I can't recommend this doctor and his practice enough!

Amazing cheek fillers in NYC

Cheek fillers 2CC of Juvederm Voluma (two syringes) by PAM LIGGETT at Plump NYC in Chelsea! 10/10 experience RESULT: Unlike when I got lip fillers, which took about two weeks for me to actually see the enhancement, this was instantaneous. I was not necessarily after more pronounced cheek bones, but rather my ultimate goal was to get a more defined, sharper, leaner face. The effect speaks for itself in the before and after picture. (Be aware that again, my face is extremely asymmetrical so it was challenging to make it perfectly balance but still so much better than before) EXPERIENCE: Went to the Chelsea location and was warmly greeted with a hibiscus tea. After filling out a quick questionnaire I was promptly taken to a private room with Pam. She talked me through the treatment and proceeded to draw lines on my face with white pen to assess the symmetry of my face and mark the injection sites. She found that my face is severely asymmetrical and therefore would need to use more filler on one side to balance out my face (something only a really talented and experienced injector would see). The injecting itself was less than 7 mins long. RECOVERY: Very mild redness which dissipated within 30 mins, and no bruising at any point. Sleeping slightly upright on my back that night was probably the only thing that was uncomfortable. The following day my face was a little sore, and smiling/laughing exacerbated it. It didn't hurt though, just sore. As far as swelling goes, I can't tell. There may be underlying swelling that will subside? PAIN: 1/10. I'm more afraid of needles than a toddler at a first doctors appointment, and I cannot stress how pain-free this procedure is. Pam was very attentive to my anxieties and made sure I was comfortable the entire time. I was also given a tool called a Vibrata to hold against my chin to mask the sound of the needle puncturing my skin (all the vibrata does is loudly vibrate). Pam is such a specialist that she replaced the needle various times since it becomes blunt. It was literally completely pain free--the only discomfort was from sitting in that chair knowing a needle was in my face. FINAL THOUGHTS: I love my new cheeks! Ive had my fair share of injections, and I can say wholeheartedly that it's not just a matter of the treatment but ALSO the skill of who is doing it. Pam is an artist with a true eye for this work. I highly recommend cheek fillers to anyone seeking to add more dimension to their face, and I implore you to book with Pam if you're in NYC.

Extremely happy with lips and botox

Hands down the best doctor that has ever tended to my lips. Im extremely careful when it comes to my face, because for one, its expensive, and for two, its my face! I I have been getting my lips done for about 8 years now, and I did a lot of research on multiple websites before visiting Dr.B. I can now safely say I'm blown away by the quality of service he delivers. My treatment with Dr.B so far has been for lip injections and botox in the forehead. He is a very good listener, which is very important to me, I need to feel that my Dr. and I are seeing eye to eye and im being understood, and he has given me gorgeous lips time and time again and I could not be happier!

Dr. Blinkski is HIGHLY recommended! He was beyond professional, informative and honest with our consultation and made me feel very comfortable, as I was very nervous about this specific procedure. During the injection he was very gentle and made sure I was not I any type of discomfort throughout, even though it did not hurt at all!
