A periareolar mastopexy will lift the nipples by max 2 cm. I dont recommend it as a procedure for you. Would recommend a formal mastopexy (auto augmentation) if you don't desire implants.
Although pictures are an inadequate medium for a surgical plan. But you have no to mild ptosis and just an implant based augmentation should be enough.
Yes, it seems to be a residual dorsal hump from the picture. The deviation isnt really visible.Would advise a consult with a certified plastic surgeon for the same.
