Botox generally starts to lose its effects at 3 months. By four months, the effects are gone. I'm not sure how long it can be kept in the refrigerator, nor am I aware of any studies that looked at this topic. However, I (an most of my colleagues in Beverly Hills) generally don't let a vial sit longer than 2 weeks.
