My experience was traumatic to say the least. I am seeking opinions from other PS. This has been a terrible experience and I just hope that all the drama will be over soon.

Hello Doctors, I am concerned about the redness that showed up at the lower pole (Not around the scar) all of a sudden 6.5 months post my breast revision. What can be the cause of it? I contacted my PS but he does not seem to be concerned, although I went to another doctor for a second opinion and he asked me to speak to my PS. The area is a bit warmer than the rest of the breast.

Must make sur it is not an infection. If the implant is infected it needs to be removed. Wait 6 months then you can put the implant back again. You need blood work, and MRI

Hi Dr. Shureih, Thank you for your response... unfortunately my condition had manifested into skin necrosis and was draining pus... although my WBC months post-op was a 4.0 . I had to have the implant removed...I am left with literally tissue paper thin skin... with a big scar across the chest wall... almost like I just had a mastectomy but with the areola and nipple intact (it is pointing down though)... A patient who had MX may have more skin than I do now just to give you an idea... What are the chances of having a successful operation again? Is reimplantation a possibilty? Many thanks for your thoughts..

I am sorry to hear it was infecuring this time the scar sorry about the scar, probably you needed a capsulectomy and that is why you have the long scar and thin tissue. You need to wait 6 month before having reimplantation. During this time the scar and and tissue will loosen up and become soft. At that time one reassess the tissue, the muscle and the skin. Yes you can have an implant again. If the skin is too tight then, one will use a tissue expander, then an implant Good Luck

Hello again I have been thinking about your situation again and have few thoughts What was the bacteria cultured. Because your infection took 6 months to manifest itself. Must know the bacteria and make sure it is not an exotic rare bacteria that requires long tem treatment,such as Mycobacteria. What was the pathology report on the tissue that was removed? must have sent the necrotic skin and tissue. Why the DELAY in diagnosis. What is hapening to the opposite breast? if you have lost significant amount of breast tissue and skin you would look like a Mastectomy patient. Such deformity will require significant reconstruction. During the peroid from the revision surgery to appearance of infection in the breast did you have any DENTAL work? Any infection any where in the body such as urinary tract infection.? When the time comes you have multiple options in reconstruction of the deformed breast. Good luck.

Hi Dr. Shureih, Thank you very much for taking the time to analyze my situation. You are so correct on the questions... I will ask my surgeon regarding the bacterial cultured. While I did have dental work done, I had it done 'AFTER' the redness appeared, and I did take a course of antibiotics before the dental work. What was suspected was the culprit was that inflammation process of the PDS sutures... as the red streaks and redness appeared along the scar line, and where the sutures were places... while some cortisone cream suggested by both my dermatologist and my PS seemed to have calmed down the redness, necrosis developed as the skin was 'atrophied' (my implant was bottomed out during these months as well), and eventually I was draining pus (two days after another well known PS had said that 'patient has no signs of infection' but he said that my skin was breaking down... The opposite breast is completely fine (for now)... it was done by another surgeon but only monocryl was used... No infection that are known in other parts of the body at all... as a matter of fact... 3.5 months post the revision surgery (i.e. right in between the revision surgery and the appearance of redness and streaks on the breast), my white blood count was a 4.0. There was never a fever either. What worries me is the options I have realistically in the future... I am 5'3 and 96 lbs... i.e. not much fat in other parts of my body...
