Rhinoplasty surgery has many objectives, and can address different areas of the nose. It is in short, a highly customizable procedure, that must be tailored to the patient's needs. It is important for you to communicate honestly and openly with your surgeon your goals, and to enquire of him to be sure that you are on the same wave length.

You have only one viewe, and it is not a standard medical photography view. With that disclaimer, I do see some fullness to the tip, some wideness to the nasal width (bones), perhaps a hump, and maybe a bit of excess columella show (the fleshy part between the nostrils from lip to nose). Again, just a cursory look.

In any event, despite these minor imperfections, you should be able to get a subtle refinement that enhances your beauty, and refines the harmony between the nose and face, without making you look weird. Remember however, your look will change even with minor changes to the nose, and that is after all why you are considering this surgery. What you really want is to change your look to the look you want!

Best to you.

A: Correcting A Bulbous Tip

C. Spencer Cochran, MD

It is possible for rhinoplasty results to appear natural as long as the nose is proportional and in balance with the other facial features. The best nose job is one in which other people cant tell that surgery has been performed.

The appearance of a "bulbous tip" most commonly arises from the shape of the underlying cartilages. If this is the case, the lower lateral cartilages (tip cartilages) can be trimmed and reshaped either with sutures or with grafts to relieve the excess curvature. Sometimes, increasing the amount of tip projection makes the nostril sidewalls straighter and can help alleviate bulbousness. Some patients prefer to maintain the overall shape of their nose, but desire a "smaller version". In some cases, this is possible.

To get a better understanding of what your nose would look like after surgery, your prospective rhinoplasty surgeon should be able to show you before and after computer simulations. Many rhinoplasty surgeons use computer imaging, and recently 3D rhinoplasty imaging has become available (see link below).
