Loose upper abdominal skin after a tummy tuck
Sometimes in patients with severe laxity, they may still have loose skin as a rebound effect above the umbilicus. You may need a reverse abodminoplasty or a vertical component or redo tummy tuck to achieve your goals.

Tummy tuck and reverse tummy tuck
This is difficult to assess and provide advice without photos. Clearly insufficient skin may have been removed but overly aggressive skin resection can result in compromise of skin circulation. This is quite often a judgment call. In my experience, individuals with prior history of massive weight loss are most prone to early skin laxity and in some instances require a revision or "reverse abdominioplasty".

Loose skin AFTER a Tummy Tuck
Skin is naturally movable and everyone in a sifting position has loose skin unless the have extremely low body fat. A picture would be helpful to see exactly what the issue may be.

Loose skin after extended tummy tuck
Without a photo it is hard to say but usually the extended TT is done on massive weight loss patients. In these folks, their skin elasticity is permanently destroyed and their skin will never be "tight" no matter what you do. Also, unless you had a vertical scar also there may still be extra skin in the upper abdomen. Check with your doctor but this may be "as good as it gets."
