Revision rhinoplasty for bulbous tip.
You will need to see an experienced rhinoplasty surgeon who can make the tip more refined but can also address any other irregularities. You may also need grafts at the nostril margin to prevent a pinched tip.

See an experienced revision rhinoplasty surgeon. It is difficult evaluate your picture, but you will probably need a reduction and refinement of the bulbous tip with improved tip support, and augmentation of any relative depression of the profile line above the tip.

A Tip Rhinoplasty Will Correct a Bulbous Nose
A Tip rhinoplasty will correct the size and relation of the lower lateral cartilages to the septum and overlying skin and correct a bulbous tip. In your case, this being a re-do operation, it would be helpful to see pictures of your original nose and the operative note of your nose surgery to see what was done and how to improve on it with your next operation.

Bulbous tip reduction
Tip volume reduction is achieve with either cartilage excision alone or cartilage excision with suture recontouring of the cartilages themselves.
Hard to tell from the picture which option you may need. However, generally a bulbous tip requires volume reduction and suture techniques.

Polly Beak Deformity
It looks like you have fullness above the tip, which creates a roundness often called a "polly-beak" deformity. The recommended correction involves removing some that tissue beneath the skin, and making sure the tip is strong enough to "project" above the line of the bridge. This can be accomplished by either open or closed techniques, depending on the patients individual needs.
