New "Tummy" Around Scar from Surgery

i had pretty major abdominal surgery 4.5 weeks ago, now have scar from belly button to pubic region. i have very low body fat, always had a flat and muscular stomach. right after surgery, i looked 5ish months pregnant! swelling has gone down steadily, now have "pooch"below naval on either side of scar. area is still a bit tender to the touch, so this might just be "healing" swelling. i am concerned it is permanent--any ideas? could this possibly be permanent, and how long does swelling last?
