Could anyone advise about this process...have not come across this anywhere else except one dr.'s website and am wondering why is this procedure not common?

Transpalpebral Osteotomy (Rhinoplasty Through the Eyelid)

I have not heard of this technique and would be wary of any rhinoplasty maneuver that involves an incision under the eyelid.

Rhinoplasty Procedure

This is not a common procedure because it has not been accepted by the medical community. New techniques are being developed all the time. However, quite honestly, to me this approach does not make sense.

Transpalpebral osteotomy

Not certain under which circumstances I would ever do this.  Osteotomies are usually performed toward the end of rhinoplasty procedure.  Even if you were getting lower blepharoplasty under the same anesthetic, it doesn't seem advantageous to leave the blepharoplasty incisions open awaiting nasomaxillary osteotomies.  Transnasal and even percutaneous osteotomy scars have never been problematic, so I am skeptical of a transpalpebral approach.

Transpalpebral Osteotmy in Rhinoplasty Surgery

Over the last 35 years I've never heard of doing an osteotomy through the eyelid skin - I would not do this. Osteotomies are usually done with an intranasal incision, but on occasion with a tiny incision through the skin covering the side  of the nose. Talk to this physician - the description on the website may not be clear.

Transpalpebral osteotomy?

I have never heard of a transpalpabral osteotomy.  I usually go transcutaneous along the outside, or through an intranasal incision.

Such a rhinoplasty osteotomy technique is uncommon, or unheard of, because it offers no advantage in the outcome of the surgery. Nasal osteotomies are done to help narrow the upper part of the nose or to close an open roof due to a significant hump reduction. Trans-Palpebral Osteotomy

Hi Arron, I don't see any advantage to this kind of osteotomy. Usually if we need to do a osteotomy so high we do a high transcutaneous osteotomy which achieves the same results. But maybe someone can make an argument for this technique, I don't see the advantage. Best, Dr.S.

Because there is not much of advantage

For any procedure become accepted it should have lower risk and more advantage than the current techniques. Some times procedures are promoted to gain publicity without any merits.

It is not really rhinoplasty through the eyelid.

Hi. One of the usual steps in rhinoplasty is to narrow the nose with in-fractures. This can be done from inside the nose, or through a tiny stab wound between the inside of the eyelids and the upper part of the nose. The other rhinoplasty steps (getting rid of bump, refining the tip, etc.) are still all done from inside the nose. see video
