I am underweight at 5'7" and 110lb. 1. Are my cheeks (especially left cheek), cheekbones, and chin underdeveloped or are my jaws just oversized instead? 2. Is weight gaining a solution to change my cheeks and chin into how I depicted in the edited photos or is a combination both weight gaining and surgery required? 3. If required, which procedures would be the most reliable in the long term to enhance my cheeks and/or cheekbones and my chin? Thank you for your advices.
Answer: Facial Contouring with Fillers (Juvederm Voluma, Vollure, Volbella, Radiesse, Bellafil), Sculptra, Fat Grafting, Implants Facial sculpting and contouring can be created with well-placed fillers (Voluma, Countour, Radiesse, Bellafill) and Sculptra, which would give improvement in facial balance. Fat or implants are alternatives. I recommend getting a formal evaluation with a facial sculpting expert. Best, Dr. Emer

Answer: Cheek and Chin Augmentation Your ethnicity accounts for your flatter midface and cheek bones. The combination of cheek and paranasal-maxillary implants would help bring your cheeks and midface further forward to put it into better balance with the chin augmentation you are demonstrating. Your chin can be advanced with either an implant or a sliding genioplasty.

Answer: Undersized cheeks and chin Dear sl765, Thank you very much for your clinical post and photographs. From review of the photos submitted, you have very pronounced malar eminence and cheekbone on the left side. There’s a lack of volume in the hypomalar under the cheekbone region. There also appears to be a degree of Microgenia or small chin. To improve your craniofacial aesthetics you didn’t mention your age, but you should wait until the age of facial skeletal maturity, which is approximately 18. There are some relatively simple techniques that can be performed such as soft tissue fillers with Juvederm Voluma or other long-lasting facial injectables that can create the type of symmetry you have shown in your edited photo. Longer term options might include facial fat grafting or facial implantation with artificial cheek and chin implants or structural bone repositioning and bone grafting. Once you reach the age of facial skeletal maturity, I would seek out the advice of craniofacial specialists if you seek bony restructuring and surgery and one of the best in Toronto would be Dr. Oleh Antonyshyn at Sunnybrook Hospital. I hope this information has been of some assistance and best of luck. For more information, please review the link below. R. Stephen Mulholland, M.D. Certified Plastic Surgeon Yorkville, Toronto
