I was wondering if lateral canthoplasty or any other sort of surgery would realistically give my downturned eyes an upward almond appearance? My outer corners are lower than the inner corners and it gives me a sad appearance. In the attached picture I have pushed my outer corner up to show roughly what I am looking for in comparison to my regular eye. Is it possible to get such drastic results? I am 20 years old if that matters.

It is difficult to see clearly on the images that you have provided but it appears you also have some globe prominence.A procedure such as a lateral canthoplasty will improve the lower eyelid contour. The long term results and maintenance of the elevated position is something that can be less predictable and there usually is a settling period over a few months after surgery. It is also important to assess the anatomy of your orbital rim to ascertain that the soft tissue and bony relationships are favourable for a canthopexy. Answer: Canthoplasty Canthoplasty is the procedure you are describing. As mentioned by the other posters there is no cookie cutter operation, especially around the eyes. You also need to know that changing the shape of your eyes will significantly alert your appearance. Have your surgeon produce imaging morphing that will given you an idea of how you would look afterwards. Often just pulling your eyelid upward does not give you a full appreciation of the change in appearance such an operation would enact. If you still desire the surgery after such a consultation, then find a surgeon who is qualified, understands your goals, and will act conservatively so as not to produce an outcome that is undesirable.
