I've Gone Through 3 Courses of Accutane, Acnes Still Around. Whats Next?

I've gone through 3 courses of accutane, and my face looks so so, but I still break out under my skin and get large lumps and white heads. A dermatologist I went to in UT told me topical medications wouldn't help since I was breaking out from the inside out, so I don't know what to do. Would taking a vitamin D supplement help? I'm a lighter skin African-American male, 19, about 150 lbs.
