Skin tightening devices that are safe and don't cause fat loss

There were a lot of problems in the past with the first generation of radiofrequency tightening devices (Thermage). I know they have been resolved, but how can patients rest assured that the newer laser treatments for skin tightening and rejuvenation (Pearl, Titan) are safe and don't cause fat loss?

A: Titan for skin tightening

Edmond A. Zingaro, MD

There were problems with fat loss from Thermage (Thermacool) treatments. This has never been the case with Titan. Thermage uses radiofrequency while Titan uses laser energy to tighten skin. We have used Titan since it was approved and we have never had a problem with fat loss. It works VERY well.

You say that fraxel is good for skin tightening, as is titan. However, I had active fx more than 2 months ago, little if any results. Does that mean that titan will be ineffective for me as well? I am 51.

TITAN skin tightening uses infrared light to heat and stimulate fibroblasts (collagen producing cells) to speed up production. Existing collagen becomes more tightly coiled and the effect is excellent. I have experienced no fat loss with this safe, FDA approved procedure, and it's much less painful than Radio Frequency treatments.

I have used TITAN by Cutera for 4 years now. I have never seen a patient with fat loss secondary with this procedure. Pearl as well does not cause fat loss as the very  long erbium wavelength does not get near  the subcutaneous tissue. I have seen fat loss in the cheeks with RF (Thermage) and unfortunately I myself had this problem. I have resolved it well with Sculptra injections. I also found the procedure more painful than the Titan. CO 2 Fraxel also is useful for skin tightening, but I like TITAN for its comfort and its no downtime aspects. Pearl has a 3-4 day social downtime, but only requires one treatment.
