These bumps appeared after a revision rhino, so I had another surgery a few months ago and they are back. Are these bossae and what is the process to get rid of them? My last doctor said he got rid of them, but they are the same size as before so why would that be? Is this hard to get rid of and do you resize cartilage, rasp the area? I am not getting answers from my previous dr.

Bossa (plural, bossae) is a term that means a palpable and/or visible bump in the nasal contour (generally the tip) which occurs after rhinoplasty. This can happen right away or years down the line. Generally, these occur when sharp angle or edges of cartilage are present in an area with relatively thin skin. Treatment can be as simple as making a tiny poke-hole and shaving the cartilage with a needle, or as complex as completely re-opening the nose and reducing or reorienting the cartilages. Determination of the best approach for you would require in-person examination.

Answer: Cartilaginous bumps on your nasal tip after revision rhinoplasty may be bossae which may be improved. I read your concerns and reviewed your up-close photos which are limited for evaluation.You appear to have rosacea with tip redness and dilated visible capillaries. You may wish to consult a reputable dermatologist regarding your skin which could be treated with IPL, laser, or light electrosurgery.Insofar as your tip irregularities are concerned, these may be irregular cartilaginous bossae which may not be improved with further surgery. You may have this skin overlying your nasal tip and firm underlying cartilage, and both of these pre-operative conditions may have contributed to your current appearance.In our practice, many of our patients have seen improvement in the appearance of bossae after Silikon-1000 injections. This is an off-label filler for permanent results. Silicone nasal injections may add volume to your tip and add a layer of cushioning between your skin and irregular tip cartilages.
