How Can I Get the Creases in the Ala Complete Flat (And That Smile on One Side of my Nose Isn't Completely There)?

I had alarplasty as part of the rhinoplasty in 2010. While I am happy with the rhino/alarplasty, I am not with the scarring left in the creases on either side. I had sutures and when removed, a few were missed and not detected until a follow up. So some of the problem are a few tiny holes in each crease but in other areas of the incision small parts of skin are not completely flat. The Dr. gave me a few injections and told me to keep rubbing the sides but it still looks kind of rough. Help.

Alar Crease Scar Touch Up

I would recommend you consider possible dermabrasion of this area to help blend the scars.

Alar scars

At now two years, I doubt that the scars will improve. It is a bit unusual for the alar base reduction scars to be a problem.

Scar after alar reduction

When well planned alar reduction scars are very difficult to see. Even if a suture was left the healing tends to be 'forgiving' in this area. You didn't mention what 'injections' you were having, but it might be best to just leave things alone for several months to see how it settles.
