My fear is so great of General or deep sedation that it is preventing me from breast agumentation. I have had three children naturally and prefer that route when ever possible. I'm wanting small implants (250cc silcione) on top of the muscle. Is there a way to do this using a local and a mild IV sedation like versed? I sure hope so because don't have the courage to go under with a general anest. drug..Please help!!!

IV Sedation Breast Augmentation

Most augmentations are done under general anesthesia in my practice. However, I would consider an IV sedation and local in certain cases. An ideal situation would be sub glandular with a small implant. There would need to be an understanding with the patient regarding her willingness to be partially awake as well. Be sure to discuss your expectations with your surgeon and their anesthesia provider.

Subglandular implants with local and Versed? What if you have problems?

No elective surgery should be undertaken with the restriction that no general anesthesia be used. You may, at some time, require more than simple implant placement, and general anesthesia may become necessary. Are you prepared for that eventuality? In fact, IMHO there is a higher chance of capsular contracture, visible implant edges or ripples, and therefore re-operation with subglandular placement. Augmentation Can Be Performed With Local & Sedation

It is certainly possible to perform augmentations under local anesthesia with sedation; in fact this the method that I performed most augmentations several years ago.  I usually use general anesthesia at this point (because there is no longer a financial incentive for local), but I am still willing to do this in patients who request it.

Local Anesthesia and Breast Augmentation

It is possible to have subglandular augmentation with only local and versed. You will experience pain and discomfort with the procedure and in some cases, the procedure may take longer because of your discomfort. Weigh all aspects of the procedure in decision: implant position, final result, look, recovery, and surgical experience.

Dear South Carolina, I have performed this operation under both conditions. Although yes it is possible to perform the surgery with only sedation, and local anesthesia, I feel that this approach can result in more discomfort during the procedure, and may take longer to perform due to the need for the administration of additional meds to keep your body from reacting from the surgical stimulus (pain).  And I am never sure how a person will do under sedation alone. Breast augmentation

The main advantage can be for the plastic surgeon who may be able to work better with the patient who is completely under general anesthesia.  Placement of the implant may be easier.  Also, if there is any inadvertent bleeding, control will be easier.  General anesthesia is very safe and easy, especially with laryngeal mask.

Local and Sedation Completely Possible For Any Breast Augmentation

It is a myth that you cannot perform a breast augmentation, even submuscular, under local and sedation, and that it is any less safe than under general anesthesia.   In my practice, I do augmentations exactly this way, whether they are above or below the mucle, need a lift at the same time, etc.  The sedation, while making patients sleepy, does not interfere with their respiratory rate, heart rate, or blood pressure.  The recovery is much easier then general... more

Implants under local

I would not recommend having implants placed this way. You would be more comfortabel under deep sedation at the minimum.  This allows the surgeon to focus on the surgery and not titrate your medications.

While this is possible it is not necessarily adviseable as Dr. Rand has suggested. This is not the reason that you should exclusively use this approach.

Breast augmentation under local anesthesia

Going above the muscle isn't as good as below for almost all patients regardless of their level of activity.   Also, an augmentation can be done under local with sedation but if you are asleep from the sedation you are actually technically under general anesthesia without your airway protected.  Not as safe as a full general with a laryngeal mask airway protecting you from not getting enough oxygen.
