Next Step After Botox and Braces? (photo)

I was diagnosed with TMJ years ago and have gone through Botox treatment, braces, and used a splint to try and fix the problem. None of these things have worked. Also, my smile has always been asymmetrical, but the braces have caused more of an asymmetry -- so much so that the doctor administering my Botox suggested giving one side more injections at my last visit because the muscle on one side was spasming more. What will alleviate my jaw pain as well as give me a more normal jaw line?
