Fat transfer and smoking
Fat transfer is really fat grafting as well. You take out live fat cells from one part of your body and place it in the face. The blood supply to the area is a major factor in determining how much of the fat will survive and last. 3 weeks is a decent amount of time after a fat transfer, you should be fine. Just remember that the results will change over time as the fat gets incorporated, the more you avoid smoke, the better the results.

Smoking cigar after fat graft
In my opinion it is unlikely that smoking half of a cigar in your third week of post operative care will be detrimental to your healing. I do advise all of my patients to quit smoking before and after their procedures as it does delay wound healing. This is more important in some other procedures in comparison to your fat grafting. Advise your surgeon and take his further advice on the matter. Best regards!

Fat grafting and smoking
More worrisome than your puffing on the cigar is your exposure to second-hand smoke that goes along with it. Smoking is felt to be very detrimental to fat grafting patients. Many surgeons will not perform a fat graft on a smoker.

Fat grafting and avoiding smoking
It is doubtful that three weeks after fat injections puffing on a cigar this once would have interfered with the "take" of the graft. The first two weeks are thought to be the most important, but try to obstain as much as possible until your surgeon says it's OK.

Cigar Smoking and Fat Grafting
I don't think that "puffing" half a cigar will significantly alter your fat transfer result - as long as it was a cigar of good quality.
Without joking, the nicotine exposure and other substances you were exposed to from the cigar should not make a noticable difference overall.

Avoid being around any kind of smoke after plastic surgery.
It is VERY important not only to avoid inhaling smoke but to avoid being around it as well. Your body needs oxygen to heal. Being around the smoke is just as bad as smoking.
