Could This Be Dead Skin or Something More Serious?

I have a strange discoloration over the entire part of my face, and have dealt with this problem for sometime now. I am not sure what it is or what could have caused it. The skin is mostly blotchy, dark, uneven, and scared. Over the past two years I've tried over the counter 2% hydroquinone creams and 1% Retin A gel to help remove the discoloration, only to achieve little to no results.
