I had a breast reduction 25 years ago. Have the report. It doesn't state if nipple was seprated. Over the 25years, have gone from a C to DD. Original reduction, had lots of excess fat on the outter sides of the breast, which has increased even more(they look smaller without a Bra on) If I proceed with a second reduction, can they improve the look drastically? The doctor wants to do lipo on the side. Is that advisable, or should I ask for the fat to actually be removed by cutting out? Any pics. from double proceedure?

Repeat Breast Reduction?

Thank you for the question. Your situation is not uncommon. it is helpful to you have the previous operative; make sure your current surgeon has a copy so that he/she knows exactly what procedure was done previously ( this may help with avoidance of complications). Liposuction of the lateral chest area is commonly done at the same time as breast reduction surgery; this should not be concerning. Best wishes.

Don't micromanage your Plastic Surgery

Hi there- Generally speaking, I think it is a grave error to get too involved in the details of the surgical technique your surgeon recommends for you.  The most unhappy patients I see are those who were able to find a surgeon willing to compromise what they would normally recommend for a patient in order to do the operation the patient thought they wanted. After surgery, most often the patient doesn't want to hear about how their requests affected the outcome- all they... more

Lipo for the lateral chest folds

It is common to perform liposuction of the outer lateral chest wall folds at the same time as a breast reduction. It should not impact the main breast reduction flaps in the majority of cases.

Liposuction of the outer breast and torso in breast reduction

Most women who undergo breast reduction benefit from liposuction of the outer breast and the adjacent torso. This helps reduce the wrap-around look of the breast, providing a more distinct border between the breast and the torso. Whether to use liposuction exclusively for the breast reduction is a decision for you and your surgeon, but I certainly think that liposuction "on the sides" is very helpful in most patients.

Second breast reduction with liposuction

Great question in surgical management. With out photos it is very hard to give exact regime. I recommendation is a two staged operative treatment plan. Stage one a full reduction being aware of the previous techniques used as so not to harm the nipple/areolar complexes. After a few months of healing, under local anesthesia a lateral liposuction if needed. Thanks for an interesting question. From MIAMI Dr. B

Secondary Breast Reduction

I think you are obsessing needlessly about the surgical details. If you present your 25 year old operative note to a qualified Plastic surgeon, who can examine you, he/she would know what to do. The technique of your old breast reduction will be apparent as will all the surgical options available to you. In the hands of an experienced breast surgeon you should have a very good result.

Think of breast reduction as sculpture.

Hi. 1) Liposuction is just a tool, like a pair of scissors. So don't worry about which tool your surgeon uses. 2) Just make sure you have a surgeon you trust, and make sure he or she understands exactly the look you want. With good surgery, a revision breast reduction should give you a very good result.

Is liposuction safe for a second breast reduction

Liposuction is a great tool for breast reduction revisions. However, it really depends on what you need removed. Some surgeons will do liposuction as the first procedure for breast reduction, but not many. Most do liposuction as a part of the breast reduction procedure (for the sides), and some do it later as a revision. It is hard to give you a good answer without actual pictures. Good luck.

Liposuction for Revision Breast Reduction

A significant advantage of liposuction (vs. excision) is that a second excision procedure puts the remaining blood and nerve supply to the nipple/areola at risk, with the potential for loss of sensation or tissue gangrene. The main disadvantage of liposuction is that it does not allow for the repositioning or tightening of the breast tissues- any lift-effect will occur ony because the breast is lighter.

Secondary breast reduction

Whether or not do use liposuction or surgical excision is an individual decision. The main advantage to liposuction is a very small incision and scar. It is not unusual, and often common, to use liposuction to supplement the surgical excision but again the technique and plan is individualized to each case and surgeon. Your comment about the technique involving separating or moving the nipple is more important because it can indicate where the main blood supply to the nipple currently is.... more
